Description
This Sources Sought Notice is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this Sources Sought Notice will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. The information provided may be used by the Department of Veterans Affairs in developing its acquisition approach, statement of work/statement of objectives and performance specifications. Interested parties are responsible for adequately marking proprietary or competition sensitive information contained in their response. The Government does not intend to award a contract on the basis of this Sources Sought Notice or to otherwise pay for the information submitted in response to this Sources Sought Notice. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The VA Heartland Network 15, Contracting Office located at 3450 South 4th Street, Leavenworth, KS, 66048-5055 is seeking a Waste Removal vendor (see Draft SOW). NAICS Code: 5612111 (Solid Waste Collection) Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Currently a total set-aside for Service Disabled Veteran Owned Small Business firms is anticipated based on the Veterans Administration requirement with Public Law 109-461, Section 8127 Veterans Benefit Act. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation will be published. DRAFT STATEMENT OF WORK Topeka Branch and Leavenworth Branch and Community Based Outpatient Clinics 1. Provide hazardous waste removal for the period of 2018 through 2023. There will be no additional surcharge for gas, administrative and environmental fees added beyond the contracted monthly rate. Contractor is to provide a copy of medical waste liability insurance. 2. Contractor shall furnish all personnel, materials, tools and equipment necessary to provide removal, transportation and disposal of all regulated medical waste and to comply with all current local, state and federal regulations. Regulated medical waste will consist of isolation waste, cultures and stocks of infectious agents and associated biological human blood and blood products, pathological waste, contaminated sharps, miscellaneous laboratory waste, discarded dressings, and contaminated linen or disposable linen substitutes. 4. VA Medical Center employees at the Eastern Kansas Heath Care System and Community Based Outpatient Clinics: Warrant that the regulated waste presented for disposal will not contain any hazardous or radioactive wastes as defined by an applicable law or regulation. VA employees will ensure all patient information is removed before placing the item in the disposal bag. Containerize and seal the regulated medical waste by placing the waste in approved plastic waste receptacle liners and/or disposable sharps containers that are marked with the international biohazard symbol and the wording, "Infectious Waste," or "Biohazard Waste." Seal the Contractor provided box filled with medical waste and transport the container to the designated storage area for pickup at the times and places agreed upon by the parties. Contractor shall: Provide the Eastern Kansas Heath Care System and Community Based Outpatient Clinics with sufficient amounts of tape, labels and collection containers of approved size and including red waste bags that are sturdy, rigid, leak proof, impervious and have the capability of being sealed. Remove regulated medical waste using only properly permitted and licensed trucks and drivers. Transport the containers by methods and procedures that fully comply with all local, state and federal rules and regulations. The Contractor shall furnish the Contracting Officer s Representative (COR) with a fully detailed description of the methodology that shall be used to transport, treat and dispose of the waste. Provide the COR with a copy of the manifest, which includes the generator and transporter information for each load of waste. The manifest shall be provided to the Department of Veterans Affairs each time the waste is removed from the Medical Center and Community Based Outpatient Clinics. Each manifest shall include, but not be limited to, the following information: GENERATOR INFORMATION: NAME, ADDRESS, TELEPHONE NUMBER, DESCRIPTION OF WASTE, DATE OF PICKUP, POUNDAGE OF EACH PICKUP. TRANSPORTER INFORMATION: TRUCK NUMBER, TRANSPORTER NAME, ADDRESS, TELEPHONE NUMBER, DRIVER NAME. DESTINATION INFORMATION: SITE NAME, ADDRESS and TELEPHONE NUMBER. The manifest shall indicate that the waste was picked up at the generator and delivered to the approved destination without incident. Within five (5) working days from the time the waste reached its destination, Contractor shall provide the Department of Veterans Affairs with a matching completed manifest that includes appropriate destruction information. The manifest will also indicate the poundage of each box picked up. Document numbers and information on the two documents shall match. There shall be final certification by the destination-authorized agent by signature and date that the waste was accepted and all the information was correct. Contractor shall maintain licenses and/or permits required by Federal, State, City and County laws and regulations. The contractor shall assume full responsibility for compliance with all Federal, State, City and County laws, rules and regulations governing regulated medical waste. 5. Training: Within 30 days of contract award, contractor shall provide a documented In-Service Training Program to participating facility employees that shall include applicable: State and Local Regulations a. Environmental Protection Agency (EPA); Center for Disease Control (CDC); Occupational Safety and Health Administration (OSHA) regulations and standards b Medical Waste Stream- policies and procedures to include: (1) Identification, segregation and labeling (2) Collection, handling and transport (3) Storage, treatment, and disposal/recovery (4) Record keeping/tracking and risk management The contractor shall obtain signatures and social security numbers of all trainees and issue certificates of completion to all trainees who have successfully completed the training. Signatures and social security numbers and copies of each certificate shall be provided to the Contracting Officer s Representative. 6. Waste Stream Analysis: Within 60 days after completion of training identified above, contractor shall conduct a waste-stream analysis of the participating facilities Regulated Medical Waste. The analysis shall be based on the policies and procedures of the Missouri, Kansas and Illinois Department of Public Health (DPH) regulations, Environment Protection Agency (EPA) recommendations, Occupational Safety and Health Administration (OSHA) standards, Center for Disease Control (CDC) guidelines, and Joint Commission recommendations. The waste stream analysis shall include any suggestions for waste reduction with the Medical Centers. SPECIAL REQUIREMENTS: 1. QUALIFICATIONS OF OFFERORS: Quotes shall be considered only from companies who are regularly established in the business called for, and who in the judgment of the Contracting Officer are financially responsible and able to show evidence of their reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service. Contractor shall be able to handle all regulated medical waste removal requirements immediately upon start of the contract period. 2. INSPECTION OF PREMISES: Contractors are encouraged to visit the participating facilities to fully inform themselves of the character and conditions under which the service are to be performed. Failure to do so shall in no way relieve the successful offeror from the necessity of furnishing the services as specified in this solicitation without additional cost to the Government. This inspection may be made daily between the hours of 9:00A.M. to 3:00P.M. except Saturdays, Sundays and holidays. 3. INSPECTION OF OFFERORS FACILITIES: The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any offeror and reject any proposal, irrespective of price, if it shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. 4. RESPONSIBILITY FOR THE CONTAINERS: The contractor agrees that the participating Government facilities assume no liability for damage that may be done to the trash containers used in the performance of this contract, unless it is caused by any act of negligence on the part of a Government employee. 5. LICENSE: The contractor shall be licensed by the state or local authority in those localities where such licenses are required. Where no license is required, the offeror shall furnish with his proposal evidence of satisfactory work performance of work supervision for a period on not less than one year previous to the date of the proposal. 6. LIABILITY AND INSURANCE COVERAGE: Satisfactory insurance coverage is a condition precedent to award of a contract. In general, a successful offeror shall present satisfactory evidence of full compliance with State and local requirements. State approved sources of insurance coverage ordinarily shall be deemed acceptable to the Department of Veterans Affairs installation, subject to timely certification by such sources of the types and limits of the coverage afforded by the sources. 7. EVIDENCE OF COVERAGE: Before commencing work under the contract, the contractor shall furnish to the Contracting Officer a certificate of insurance indicating the coverage outlines in the SUPPLEMENTAL INSURANCE REQUIREMENTS section and containing an endorsement to the effect that cancellation of or any material change in the policies which adversely affect the interest of the Government in such insurance shall not be effective unless a thirty (30) day advance written notice of cancellation or change is furnished to the Contracting Officer. 8. AUTHORITY OF OPERATORS: The contractor s operators shall bear at all times the contractor s signed authority designating them as authorized to perform work at the participating facilities under this contract. Only designated operators shall do work under this contract. All drivers of vehicles used in the performance of this contract shall be authorized drivers of such type vehicles and shall carry upon his/her person the necessary operator s and/or chauffeur s license as may be required by the States of Missouri, Kansas, Arkansas. While driving on Government facility grounds, the drivers of vehicles shall fully comply with such speed and safety regulations pertaining to the operation of motor vehicles as medical authorities may issue from time to time. 9. SAFETY/EMERGENCY PLANS: Contractor shall submit the following plans with their quote: a. A written safety plan that details actions to be taken to protect the staff of the medical centers, contractor, and surrounding communities in the event of any accident spills or leaks that occur while the contractor is performing work under this contract for the Government. b. A written emergency plan that outlines a formalized response to any accident spills or leaks that occur while the contractor is performing work under this contract for the Government.
