- The Amarillo VA Health Care System (AVAHCS) is seeking Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 12. NOTE: TO QUALIFY SDVOSB AND VOSB VENDORS MUST BE CERTIFIED IN THE VA VENDOR INFORMATION PAGES (VIP) DATABASE. The North American Industry Classification System (NAICS) is 334516. Any SDVOSB or VOSB firms who wish to identify their interests and capability to provide this product must provide product specifications, performance and delivery information by notifying the Contract Specialist no later than NOON PM Central Time, July 5, 2018. Notification shall be e-mailed to Robyn Nussbaum, at robyn.nussbaum@va.gov. Any vendor who responds to this Notice must provide credentials to provide quality control materials as outlined in the below scope. All information received in response to this Notice that is marked as proprietary will be handled accordingly. In accordance with the Federal Acquisition Regulation, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Notice. I. SCOPE: The intent of the Amarillo Veterans Affairs Health Care System (AVAHCS) Pathology & Laboratory Medicine Service is to provide accurate laboratory testing and to our Veteran patients and in doing so AVAHCS is requesting the procurement of Quality Control (QC) material in liquid or dehydrated form to validate the precision and accuracy of the laboratory results being generated on the various instruments and that it is compatible with the QC software program that is currently installed to monitor progress. AVAHCS is also requesting the purchase of annual subscription services to provide real time statistical evaluation of laboratory quality control material which is currently in use as well as connectivity support. II. BACKGROUND AVAHCS is required to participate in inter-laboratory quality control programs (IQAP) with other laboratories utilizing the same instrumentation and lot numbers of reagents by processing the quality control material. The QC data is generated by processing the QC material through the instrumentation as though they were actual patients being tested. This data is then sent through a software program known as Bio Rad Unity Real Time. This software program can track QC material results being processed on a day to day basis, collate the information, submit this data to the quality control manufacturer via e-mail, and the results, following data reduction, are emailed back to the laboratory. The QC software program currently in use allows the laboratory staff to apply statistical rules to the QC data reports and assess them against other laboratories in a real-time environment using the same QC material and the same QC material lot numbers. The primary purpose of the results comparison is to closely monitor the performance of the laboratory analytical process and to ensure valid results are being released to the patient s charts. Quality control material evaluation is mandated by both of our accrediting agencies; the College of American Pathologists (CAP) and the Joint Commission (TJC). III. QUALITY CONTROL MATERIAL REQUIRED Refer to Attachment A and Attachment B for list of quality control material and an annual estimated inventory usage for Amarillo VA laboratory and the Lubbock Community Based Outpatient Clinic (CBOC). IV. VENDOR RESPONSIBILITIES The Vendor shall provide, in accordance with all applicable federal, state, and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, supplies negotiated on GSA contracts to enable AVAHCS to accurately and efficiently monitor the analytical processes of the instrumentation used throughout the laboratory. The Vendor shall provide the ability to automatically ship reagents monthly, quarterly, or on an as needed basis to the Amarillo VAHCHS P&LMS and to the Lubbock CBOC. The Vendor shall provide the ability to sequester lot numbers in such volume as to maintain the same lot number of quality control material throughout the Period of Performance or longer. The Vendor s Quality Control material shall be compatible with the Bio Rad Unity Real time QC software program that is currently installed in Amarillo and Lubbock laboratories. The contractor shall provide annual subscription services and connectivity support to the current software program that has been installed at the AVAHCS located in Amarillo, TX and Lubbock, TX. (Refer to #6 below for address) The contractor shall provide all of the necessary equipment to perform and use the Bio Rad Unity Real Time software in both locations as noted in #6. Annual subscription service for upgrades and enhancements will occur at the beginning of each fiscal year or as needed for both facilities to stay compliant with accreditation agencies. The general information to be included on the quality control data reduction manifest shall state the following: Facility Name, address, and phone number Type of service performed Date of service Signature verifying the information and approving the disposal site to be use. d. The testing data information to be included on the quality control data analysis shall state the following: Monthly evaluation report Instant QC reports Laboratory performance overview report Laboratory comparison report Laboratory Histogram report Bias and Impression histogram report Data rejection report Worldwide comparison report Statistical profile report to include: The 2 and 3 standard deviation ranges for our laboratory Summary statistics Histograms Percentile distribution tables Service and/or Maintenance of the QC software program known as Bio Rad Unity Real Time shall be provided at no cost and 24/7 via telephone troubleshooting and shall onsite service be required; the contractor is to contact the Contracting Officers Technical Representative (COR) for access hours. Normal work hours for both the Amarillo and Lubbock locations are 8:00AM to 4:30PM. The Vendor shall deliver/ship the quality control material/supplies along with annual subscription and connectivity support to: a. Amarillo VA Health Care System b. Lubbock CBOC 6010 Amarillo Blvd., West 6104 Ave Q South Drive Amarillo, TX 79106 Lubbock, TX 79412 Attention: Laboratory Attention: Laboratory The billing invoice must include the Period of Performance (POP). The Vendor shall provide the facility with the most current Food and Drug Administration (FDA) approved reagents necessary for the testing of blood and blood components. All requirements and provisions defined in the specification of this solicitation will apply to any vendor, i.e., branch, division, sub-Vendor, etc. supplying parts or reagents on behalf of the Vendor. For quality control material with hazardous constituents, the Vendor shall provide a mechanism for the Laboratory to meet local discharge requirements. The Vendor shall supply information regarding any hazardous chemicals. V. LICENSING AND ACCREDITATION The Vendor shall have all licenses, permits, accreditation and certificates required by law and/or regulatory compliance. VI. SECURITY The C&A requirements do not apply, and that a Security, Accreditation Package is not required.
