Description
The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 2), James J. Peters VA Medical Center, Bronx New York, intends to award a contract for Sizewise LTC hospital beds the Veterans Affairs, Northport VAMC, 79 Middleville Road, Northport, NY 11768. This action will result in a Firm-Fixed Price contract. This source sought notice is not a request for quotations; interested parties may express their interest by providing a capabilities statement NLT Tuesday July 10, 2018 at 2:00 PM EST. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required supplies and services. Verbal responses are not acceptable and will not be considered. When responding to this announcement, respondents should refer to Announcement 36C242-18-Q-9429- Sizewise LTC Hospital Bed Sources Sought Notice. If after July 10, 2018, no ACCEPTABLE responses have been received in response to this announcement, the Department of Veterans Affairs shall negotiate solely with Sizewise LLC. NAICS 339113 Furniture, Hospital, Specialized (e.g. Hospital Beds, Operating Room Furniture) Manufacturing NAICS 339113 is on the SBA Class Waiver List. However, NAICS 339113 is not in compliance with the Non Manufacturer Rule (NMR) because this is a brand name only request. This sources sought notice is not a request for competitive quotes. Upon collection of all written responses received, there will be a determination made as to the most appropriate solicitation approach for this Northport VAMC requirement. The Government anticipates the award of a firm-fixed- price contract. A determination by the Government whether to compete this proposed requirement will be based upon responses to this sources sought notice and will be solely within the discretion of the government. All written responses and information received will normally be considered solely for the purpose of determining whether to conduct a competition for this Northport VAMC requirement. Objectives/Requirements GENERAL This is a non-personnel services contract to provide medical surgical and bariatric patient bed frames and bed surfaces for the Northport VA Medical Center. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Community Living Center Beds replacements as defined in this SOW. There will be no furnished property and services. The Contractor shall perform to the standards in this contract. 1.2 Period of Performance This is a onetime purchase with equipment and services to be delivered with-in 45 to 90 days of award. 1.3 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. Badges must be worn at all times. Authority: The Award will be pursuant to the terms and conditions of the Contractor s Open Market FAR Part 12 Contract or GSA - FSS Contract and FAR 8.404(b)(4). PART 2 REQUIREMENTS Description of Requirement: (Brand Name) All equipment, surfaces, and supplies will be new and unused. This combined synopsis & solicitation notice is a request for competitive quotations; therefore, all capability statements or quotations received prior to the RFQ close date will be considered. Sizewise implementation Services shall include, but are not limited to, a project manager, a detailed project timeline with defined roles and responsibilities (including Sizewise provided and facility resources), on-site installation coordination of all equipment and accessories, a training plan including product competency guides, and schedule for Go-Live. Testing of 37 PIN Nurse Call/Headwall System for High and low Priority Alerts for Bed Exit, Protocol Timer, Nurse Call, CPR and Room Environment Controls. Services will be completed with VA Engineering Staff for all deliverables. Delivery and Installation Services: Sizewise equipment, parts, and accessories as specified in the SOW will be new materials. Delivered materials to the job site in Sizewise original unopened containers, clearly labeled with name, equipment model and serial identification numbers, and Purchase Order (PO) number. The vendor is responsible for inventorying materials prior to delivery to the Northport VAMC to check for accuracy in quantity and part number. The facility shall identify and provide limited daily storage of vendor parts/supplies at the time of delivery and reject items that do not conform to this requirement. All on-site installation services shall be vendor-provided by technical staff (not sales reps) fully trained to service and install bed products, parts, and accessories as specified in the SOW. Installation Team will include certified MedTechs, Operations Specialists and National Operations/Installation Team. Disposal of Waste: Awarded services include: provide, maintain, and dispose all material waste and packaging associated with the beds/products and/or waste generated during the installation services. Responsible for moving waste materials from job site(s) to a vendor-provided waste container, and then off site. Technical Support: Sizewise offers 24/7/365 hour phone technical support and will supply support services for the duration of ownership of the equipment Facility Engineering Technical Training: Sizewise provided technical training for overall maintenance and service of Northport VAMC technical staff to include factory service training courses. Engineering staff shall receive the training. Training shall include factory based training, on site, by national and local operations teams training, installation and in-servicing teams. Service and Operator Manuals: Documentation for the Sizewise Bed Systems shall include: Two copies of the operator s manual for beds purchased. Two copies of complete technical service manuals including troubleshooting guides, necessary diagnostic software and equipment, schematic diagrams, and parts lists for every beds purchased. Electronic versions of both operators manual and technical service manuals Abbreviated operating instructions included on all beds. DVD or web based training for both operator s manual and service if available. The bed operator s manual must specify the mattress size to reduce risk of patient entrapment between bed and side rail. On-Site User Training: Upon delivery of the beds, qualified instructors must provide user training for staff on day, evening, and night shifts at times to be determined by the facility. Training must be offered 5 days a week for the first two weeks. Ongoing training annually as requested. The vendor shall provide one (2) copies of the operating manual to include schematics, installation, operation and maintenance and any other applicable manuals for system. Documentation shall be provided in English in hard copy and/or CD/DVD ROM. Vendor is to provide training and written instructions, procedures, and documentation necessary to properly and safely disinfect the equipment and accessories between patient uses PART 3 INSPECTION AND ACCEPTANCE: The contractor is expected to comply with all required Federal, State and Local laws, licenses and regulations. It is the responsibility of the contractor to be in compliance and current with all applicable laws and licensing requirements. The government will inspect and accept goods and may perform operational tests to ensure the equipment performs in accordance with the original equipment manufacturer (OEM) specifications. All beds will be accompanied with inspection verification document. Delivery shall be FOB destination to: Northport VAMC Obligation of Funds: The award is obligated only to the extent of the authorized orders actually listed under the negotiated award.
