Kingsbridge Road Rm. 9b16 Bronx NY 10468 Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Brooklyn VA Medical Center, is conducting a market survey for qualified firms capable of providing the following services for Bronx VAMC. Objectives/Requirements New York Harbor Healthcare System is requesting decommissioning, relocation, and delivery of our BD blood culture machine from the Microbiology Laboratory in Brooklyn to the Microbiology Laboratory in Manhattan. New York Harbor Healthcare System is requesting decommissioning, relocation, and delivery of our Vitek 2 60 System from the Microbiology Laboratory in Brooklyn to the Microbiology Laboratory in Manhattan. CONTRACTING TASKING/REQUIREMENTS The Contractor will be provided: Decommissioning and preparation of system for transportation from Brooklyn VA Medical Center. Transportation of system to its new location at Manhattan VA Medical Center. Reinstallation of system, calibration, and verification of system operation to factory specifications. 100% insured for damage as a result of the move. All move activities for this system must occur during normal business hours. MOVING SERVICES ACTIVITIES The process for moving activities could include, but not be limited to: Padding, packing, and/or crating as required for safe transport. Loading and transporting to the new location. Unloading and placement of the property as directed by the COR or as shown on the Government-furnished concept plan. Removal of packing and crating materials and cleanup of moving activity area as directed by the COR. Any other service which would normally be included in a complete moving activity. The contractor shall in all cases do a pre-inspection survey to verify amounts, quantities, types of materials to be moved so that there is no difference of opinion about the moving resources or moved equipment at a later date. SPECIAL MOVING ACTIVITIES Specialized moving activities include, but are not limited to medical equipment which may involve disassembly and reassembly at the interim and final locations. SPECIAL HANDLING MOVING ACTIVITIES Special handling moving activities include, but are not limited to: The Contractor shall ensure that crew members have the technical training in assembly and disassembly of equipment before performing such tasks. The Contractor shall ensure all moving service activities are performed in a safe manner. The Contractor shall be responsible for the protection of Government property. The Contractor shall be responsible for damage to property and insured to repair and/or replace damaged property and personnel. SPECIAL PROTECTION ACTIVITIES The Contractor shall provide materials for anchoring system and protectant. The Contractor shall shield all electrical outlets, closets, cabinets, wall affixed electronics and equipment from dust and debris GOVERNMENT INSPECTION/ACCEPTANCE OF ITEMS The NYHHS COR or designee will conduct a final walk through/ inspection with the contractor to ensure that the designated area is returned acceptable conditions. The contractor will correct deficiencies at no additional cost to the government. GOVERNMENT-FURNISHED MATERIALS, EQUIPMENT AND SERVICES The Government shall not furnish any materials, equipment or services. Responses to this notice must be submitting in writing via email and must be received no later than July 02, 2018. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 811219. Qualified offerors shall be experienced in addition to providers (sellers) capable of supporting this facility. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, and examples of similar facilities which you have provided services to DUNS number, address, and point of contact.
