Description
DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K) Acquisition Plan Action ID: VA261-18-AP-5556 Contracting Activity: Department of Veterans Affairs, VISN 21, San Francisco VA Health Care System, 4150 Clement Street, San Francisco, CA 94121 has an emergency requirement for stand-alone manual cassette printer with software. The acquisition number is 662-18-2-096-0047. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The stand-alone manual cassette printer with software is needed for the clinical care of patients. The cassette printer prints unique patient identification numbers on cassettes to process histology and cytology specimens. The estimated amount is $10,220.15. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This Justification and Approval is pursuant to Federal Acquisition Regulation (FAR) 6.302 Unusual and Compelling Urgency. The lab s cassette printer is broken, and the lab is unable to print the unique patient identification numbers on the cassettes for the processing of histology and cytology specimens. The new cassette printer is needed immediately, and is used to identify, diagnose, and report patients specimens and other information accurately. Description of market research conducted and results or statement why it was not conducted: The VIP Database was researched for vendors who could provide cassette printers, but none were found. Also, the acquisition package was reviewed. The same or similar items were found with pricing of $22,384.55 for a Tissue-Tek Smart Write cassette printer and $10,467.40 for a Smart Write slide printer. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
