Description
This Sources Sought Notice is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this Sources Sought Notice will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. The information provided may be used by the Department of Veterans Affairs in developing its acquisition approach, statement of work/statement of objectives and performance specifications. Interested parties are responsible for adequately marking proprietary or competition sensitive information contained in their response. The Government does not intend to award a contract on the basis of this Sources Sought Notice or to otherwise pay for the information submitted in response to this Sources Sought Notice. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The VA Heartland Network 15, Contracting Office located at 3450 South 4th Street, Leavenworth, KS, 66048-5055 is seeking a Waste Removal vendor (see Draft SOW). NAICS Code: 624190 (Other Individual and Family Services) Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Currently a total set-aside for Service Disabled Veteran Owned Small Business firms is anticipated based on the Veterans Administration requirement with Public Law 109-461, Section 8127 Veterans Benefit Act. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail: to Daniel.Thomas6@va.gov. All information submissions should be received no later than 12:00 pm CST on July 25, 2018. After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation will be published. DRAFT STATEMENT OF WORK Employee Assistance Program (EAP) 1. Scope of Services: The Contractor shall provide an indefinite delivery/indefinite quantity requirements contract for providing employee assistance services for employees of the Veterans Affairs Eastern Kansas Health Care System (VAEKHCS). Services include providing employee assistance in dealing with life at work and at home, provide short term counseling, problem identification assistance, problem resolution assistance, family counseling and will maintain confidentiality of employees who request these services. Family counseling is considered to be dependents and family members as eligible if they meet the dependency and familial relationships that are recognized by the Internal Revenue Service (IRS). 2. Services provided to Eastern Kansas Health Care System (Colmery-O Neil VA Hospital & Dwight D. Eisenhower VA Hospital) (hereinafter referred to as VAEKHCS) shall include: 3. Communication in the hospital for employees such as posters, orientation meetings, benefits fairs, including promotional in-services. 4. Initial assessment and referral for: (1) Psychological counseling (2) Relationship counseling (3) Marital and family counseling (4) Alcohol and substance abuse counseling (5) Stress, job or personal 5. The contractor shall be physically accessible in local and outlying areas, within a 30 mile radius of Leavenworth, KS and Topeka, KS. Appointment times should be during normal business hours and some evening hours. Crisis accessibility shall be twenty-four hours a day, including weekends and holidays. The contractor shall respond to non-emergency requests within three business days. In emergency situations, the contractor shall provide same-day appointments. 6. The contractor's staff shall have a master's level Ph.D. credentials and previous experience in a wide range of personal problems such as alcohol and drug counseling, marital and family related issues, and experience/expertise in resolving workplace problems and delivery employee assistance and related managed care services. 7. The contractor shall furnish proof of current liability insurance in an amount not less than $1,000,000 per occurrence. 8. The contractor shall provide the Contracting Officer s Representative (COR) with quarterly reports on usage trend for management that may help identify possible problem areas with VAEKHCS. 9. A fee per hour will be paid as a fee per referral and will be paid by purchase order within a month following receipt of invoices. 10. No information shall be provided to any person/persons, agency/agencies, in or out of the Federal Government which might identify an employee or a member of his/her family without a voluntarily signed release of information by the employee or his/her family member (or legal guardian if a minor) specifying to whom, and for what reason, the information is to be released. The Contractor shall establish a confidential record-keeping system which is in compliance with Federal standards, and with requirements relating to the confidential nature of patients records in accordance with Section 333, Public Law 91-616 (as amended by Public Law 92-282), Section 406 of Public Law 92-255, and Federal regulations of the Special Action Office for Drug Abuse Prevention (21 CFR 1401). 11. General Requirements: a. Evidence of Insurance Coverage. Before commencing work under this contract, the contractor shall furnish to the Contracting Officer, a certificate of insurance indicating the coverage outlined and containing an endorsement to the effect that cancellation, of or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation of change is furnished the contracting officer. b. A monthly report shall be submitted to the Contracting Officer and Contracting Officer s Representative detailing all calls received, when they were received, emergency or non-emergency and contractor response. The report shall be sanitized so that no personal information is revealed to protect the privacy of the patient.
