- Statement of Work for Dorm Furniture PERFORMANCE WORK STATEMENT FOR DORMITORY 7220 FURNITURE REPLACEMENT DESCRIPTION OF SERVICES CONTRACTOR RESPONSIBILITIES: The contractor shall provide all personnel, equipment, tools, supervision, and other items and services necessary to replace current furnishings with new high quality and durable furnishings which consists of Captains Beds, Dressers, Night Stands, Desks, and Desk Chairs for 64 private resident's rooms. The contractor will deliver, off load, assemble, install and arrange all requested items at Dormitory 7220. Once the old furniture is removed from the resident's room, it will be immediately replaced with the complete new set of designated furniture. The contractor will remove and dispose of all identified furnishings and appliances off base, the dumpsters on base will not be utilized. The furniture to be disposed will be identified on a list which will be provided by 7 CES, Dormitory Management prior to the arrival of the contractor. The contractor will not cause any damage/modification to the facility. The contractor will clean up all debris from the work sites caused by the contractor as they go. The contractor will be accompanied by a military escort when working in occupied resident rooms. Schedule. The following schedule will be followed for furniture installation. Day One Day Two Day Three Day Four Day Five Day Six 11 Even Number Rooms 3rd Floor 11 Odd Number Rooms 3rd Floor 11 Even Number Rooms 2nd Floor 11 Odd Number Rooms 2nd Floor 10 Even Number Rooms 1st Floor 10 Odd Number Rooms 1st Floor GOVERNMENT RESPONSIBILITIES: The Government will provide reasonable utilities, common office area equipment for the contractor to perform the requested duties. Escort/Quality Assurance. 7 CES will provide an escort and quality assurance POC who will accompany the contractor to all rooms/locations where furniture is being removed and installed. The quality assurance member will inspect every piece of furniture installed and ensure there is no damage or missing items. If any damage is caused to any furniture, resident belongings or government property, it will be immediately reported to the quality assurance POC as soon as it occurs. Occupied Rooms. All resident rooms that are occupied at the time of delivery will have the resident or a designated representative present to ensure security of all personal belongings. The residents will be notified by 7 CES as soon as a delivery date is established so the residents can prepare their rooms for the furniture transfer. Elevator Available For Use During Installation. No Building Specific Information. Truck will be able to get as close as 30 feet of outside building. No elevator. A sky lift may be utilize to place furniture on floor onto roller cart. Rooms are accessible from the outside. Facility has a total of 22 units on floors 2-3; 20 units on floor 1, and Day Rooms and Laundry Rooms on 1-3 floors. Ground Floor has 10 units on each side, plus floors 2-3, 11 units on each side (front & back). Truck access limited to front side of facility given easy access to 20 units on ground floor, all items for delivery to front of Ground Floor will have to be loaded on cart and manually taken to front side of building. Sky lift/forklift may be utilize. Staging Area. The Dormitory Management storage warehouse, Building 7008, can be utilized as a staging location for the purposes of assembling and preparing furniture. Toilet Facilities. Use of existing public toilet facilities in the immediate work area will be permitted. Private resident bathrooms will not be used at any time. The contractor will ensure facility cleanliness is maintained at all times to the extent of cleaning up after contractor personnel only. Security Police and Fire Protection. The government will provide general on-base security police and fire protection service. Security police phone extension is 696-2131 for emergencies, and 696-4100 for routine calls. Fire protection emergency phone is 117 (from on-base phone only), or 696-2486. Telephone. The contractor may use government phones for the performance of duties covered under this contract PWS. No long distance telephone calls will be authorized. Utilities. All reasonable quantities of electricity will be made available to the contractor without charge. GENERAL INFORMATION. Hours of Operation. Normal working hours for the contractor will be between the hours of 8:00 A.M. and 4:30 P.M, Monday thru Friday excluding Saturdays, Sundays, Family/Goal Days, and federal holidays. Work Schedule Change Request. If the contractor needs to work during periods other than the normal working hours stated above, the contractor shall make his/her request to the contracting officer three days in advance of his/her intention to work. The altered work schedule shall not be implemented until authorized in writing by the contracting officer. Base Closures. Work scheduled but not accomplished because of base closures due to weather, exercises, or actual alert will be accomplished as soon as possible after reopening the base. Base Passes and Identification. The Contractor shall obtain base passes and identification cards for all employees and vehicles. Application will be coordinated through the Contracting Officer or designated representative. The contractor shall return contract passes of terminated employees within 24 hours of termination. The contractor shall comply with all requirements and procedures in accordance with AFFARS 5352.242-9000, Contractor Access to Air Force Installations. Safety. Contractor shall comply with all applicable OSHA and AFOSH standards. Contractor shall not bring any hazardous material onto Dyess AFB or use hazardous materials in any Dyess Facility. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned or privately-owned vehicles while on Dyess Air Force Base. Location of Works. This PWS is applicable to the following locations contained on Dyess Air Force Base, TX: Bldg 7220, 410 Ave. D. Personnel. The governments is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employees, who is identified as a potential threat to the health, security, general well-being, or operational mission of the installation and its population. Contractor personnel shall present a neat appearance.
