- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is W9124E-18-Q-0005 and is issued as an invitation for bids (IFB), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219 with a small business size standard of $20.50M. This requirement is a [ Small Business ] set-aside and only qualified offerors may submit bids. The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2018-07-20 15:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com. FOB Destination shall be Ft Polk, LA 71459 The MICC Fort Polk requires the following items, Brand Name or Equal, to the following: LI 001: Brand Name Or Equal The contractor shall provide pricing for twenty (20) brand new (never used, no grey market goods) APC 031-000009-000 L-A 12V 373W/CELL PLYROP or equivalent batteries (in accordance with FAR 52.211-6 Brand Name or Equal with the following salient specifications; length of 12.05 inches, width 6.85 inches, height 8.80 inches, weight 74 pounds, 86 AH Jar, 12 Volt lead acid battery with 373 watts per cell, 20 cells, and maintenance free) for each of the two (2) UPS systems (for a total of 40 each batteries). The batteries shall be brand new and shall be fully compatible with the existing Galaxy 4000 UPS, to include software operation (electrical status, charging status, monitoring, reporting, and testing). Each is defined as each battery. Batteries shall be delivered to FT Polk FOB Destination by No Later Than 27 August 2018. The delivery address shall be provided by the Government upon any award., 40, EA; LI 002: UPS Battery Replacement Job is defined as complete removal and replacement of 20 each batteries (from Line Item 0001) in one each Government designated Galaxy 4000 uninterruptible power supply in accordance with the Statement of Work at the Bayne Jones Army Community Hospital at Fort Polk, LA. There are a total of 2 each Government designated Galaxy 4000 uninterruptible power supplies total for this requirement (as seen in the quantity information for this Line Item). The unit pricing shall represent the contractor's price to replace the batteries (from Line Item 0001) in 1 each Government designated Galaxy 4000 uninterruptible power supply. This is a Firm Fixed Price that shall include all costs to the Government in accordance with the Statement of Work (except for costs captured at Line Items 0001 and 0003). The costs includes all travel, overhead, G&A, profit, and any other costs for successful completion. See the included Statement of Work for all requirements on this Line Item. The Period of Performance shall be from 28 July 2018 to 21 September 2018 upon award., 2, Job; LI 003: Disposal of Old Batteries This Line Item is for the disposal of 40 each batteries that the contractor is responsible for removing under Line Item 0002 and the Statement of Work. The contractor shall ensure that all batteries are transported in accordance with the Department of Transportation requirements, and are ultimately disposed of in a manner that are in accordance with the Environmental Protection Agency's regulations. If the contractor utilizes a facility or another entity for ultimate disposal the contractor shall ensure the requirements of this Line Item and the Statement of Work are strictly followed. The unit pricing is for any transportation and battery disposal (and any other costs associated) of all 40 each batteries. See the included Statement of Work for any additional requirements. The Period of Performance shall be from 28 July 2018 to 21 September 2018 upon award., 1, EA; LI 004: Contractor Manpower Reporting CMRA Reporting Contractor is required to provide data on Contractor labor hours (including subcontractor labor hours) for performance of this contract IAW the PWS. The cost, if any, for providing this data shall be entered into the space provided at this CLIN. If no direct cost is associated with providing the data, enter No Cost . Instructions, including the Contractor and Subcontractor User Guides, are available at http://www.ecmra.mil. In order to determine if the proposed cost for Contractor Manpower Reporting (CMR) is fair and reasonable, it is estimated that a onetime set up of XML schema is approximately 20 hours per contractor. Thereafter, the annual reporting requirement should take less than a few hours annually by an administrative level employee. The contractor has the option to separately price this requirement or include this cost in their overhead rates of Line Item 0001. The reporting shall be completed no later than 31 Oct 18. UIC: W2NKAA COMMAND:US ARMY MEDICAL CMD (MEDCOM) PSC: J059. The Period of Performance shall be from 28 July 2018 to 21 September 2018 upon award., 1, EA; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, MICC Fort Polk intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. MICC Fort Polk is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids. All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com. There is no cost to register, review procurement data or make a bid on www.FedBid.com.
