Facilities Management Service Purchase Request 663-18-3-211-0457 Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This requirement is to provide Puget Sound Healthcare System Gastroenterology Department with repairs and maintenance to a scope that is leased from Olympus America Inc. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Seattle VAMC is currently leasing Olympus Scope GF-UE160-AL5 SN: 1111920 from Olympus. Olympus America Inc, has proprietary rights of their equipment, and is the only authorized vendor to preform repairs and maintenance, and calibration of their scopes. Description of market research conducted and results or statement why it was not conducted: A Olympus America Inc, retains full proprietary rights to this technology. No other service provider is authorized to calibrate this equipment. No other company is authorized to perform the repair without voiding the warranty. Reviewing the market for similar maintenance indicates that retaining rights to this type of system is common within the industry. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
