- Description of Work Culvert Replacements are s located on FSR 4 Mulky Gap Road, beginning at the intersection with FSR 88 at the Dixon Creek crossing continue north on Mulky Gap Road for about.85 miles to the second culvert where Mulky Creek Crosses FSR 4, Mulky Gap Road. The road is located on the Blue Ridge Ranger District of the Chattahoochee-Oconee National Forests in Union County, Georgia. The project is located on Mulky Quad map. The project removes the existing Dixon Creek, 15 foot long by 42 inch diameter culvert and the existing 36 inch diameter 22 foot long Mulky Creek Culvert. Both culverts have CCC constructed stone head walls. The stones from the head walls are to be reserved and reused to delaminate the edge of road over the new culvert installations. The road bed is width at the culverts is 14 feet with 2 feet of shoulder. The approach road has a ditch on upland side of the road that is 1 foot deep. The approach sections are 30 feet long and have a riprap section 24 inches thick topped by 4" compacted in place of GDOT # 4 aggregate. Any of the salvaged headwall stone that is not used in the rebuilding of the new culvert headwalls, if two feet minimum diameter boulders shall be placed on the edges of the travel way to act as curbing on the culverts. The rest may left stockpiled on site for future use by the Forest Service. The replacement culvert at Mulky Creek is a Contech Aluminum Structural Plate open Box Culvert No. 43, R3 with HG/CG =2/2 or approved equivalent. Culvert spans 19' 6" and rises 6' 1" and a length of 22.5 feet. It has an opening of 97.1 ft. sq. with min. cover of 1.4 ft. and max. cover of 5.0 feet. The loading is HL-93. The head wall shall be reinforced concrete rather than the standard aluminum because of the need to replicate the masonry headwall on the new culvert. The concrete headwall measures approximately 22' 3-3/4" long and 9'-1" high with two sloping wing walls 10 feet long. The replacement culvert at Dixon Creek is a Contech Aluminum Structural Plate open Box Culvert No. 28, R3 with HG/CG =2/2 or approved equivalent. Culvert spans 14'-10" and rises 4'-10" and a length of 22.5 feet. It has an opening of 63.2 ft. sq. with min. cover of 1.4 ft. and max. cover of 5.0 feet. The loading is HL-93. The head wall shall be reinforced concrete rather than the standard aluminum because of the need to replicate the masonry headwall on the new culvert. The concrete headwall measures approximately 18' 1- ¾" long and 9'-1" high with two sloping wing walls 10 feet long. The Contractors shall provide shop drawings for the headwall and footing design with the Forest Service reviewing and approving the shop drawings before construction of the headwalls begin. Limited clearing is required at that site. Only those trees which must be removed to adjust the alignment of the new culvert and road widening are expected to be removed. The timber will be marked by the Government prior to removal. The timber will be topped, limbed, cut to length and decked in the dispersed campsite adjacent to the worksite. The tops and limbs will be used as brush barriers to protect the work site from sediment transport. Camping There are two Forest Service campgrounds in the area where the Contractor may make reservations and camp for two weeks. The contractor may also use a dispersed campsite near project site with site selection agreed upon with the Contractor and wishes to camp for longer than two weeks the Contractor may will obtain a permit from the District Ranger to use the location for duration of the contract.   B. Location The culverts are located on FSR 4, Mulky Gap Road. Access to the road is off of Georgia 60. The reach FSR 4 take Georgia 60 to Cavender Gap road approximately 7 miles from Suches Ga. Turn on Cavernder Gap Road and follow it until the intersection with FSR 236 to the left. Continue on FSR 236 approximately 3.0 miles to the intersection with FSR 4. Turn right on to FSR 4, for approximately 1.3 miles to the Dixon Creek crossing and then another.85 miles to the Mulky Creek crossing C. Environmental The contractor shall be responsible for keeping all silt and construction debris from leaving the construction limits. Any silt or construction debris found outside the construction limits shall be cleaned up and removed at no cost to the government. The Contractor shall comply with the State of Georgia erosion and Sediment control measures. Anyone who works with the erosion control best management practices (BMP's) must be licensed according to the Georgia Soil and Water Conservation Commission Requirements. D. Pre Bid Meeting The pre meeting shall be mandatory and shall meet at the intersection with FSR 4 and FSR 88. This is where Dixon Creek crosses Mulky Gap Road. E. Price Range less than $500,000 F. Contract Time The Contractor shall have 180 days to complete the contract the project. The contract shall run from. The Contract time shall begin 45 days from the date of award. G. Inspection and Acceptance Periodic field inspections will be made by the Contracting Officer's Representative (COR) or the COR's designated inspector. Work not completed to the Contract Specifications shall be reworked without compensation to the contractor. C2 SPECIFICATIONS The Contractor shall furnish the necessary personnel, material equipment, services and facilities (except as otherwise specified), to perform the description of work/Statement of Work reference in Section J. The projects specifications are included by reference are Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects FP-14. They may be obtained on the internet at https://flh.fhwa.dot.gov/resources/specs/ SECTION M--EVALUATION FACTORS FOR AWARD EVALUATION FACTORS The following factors will be used to evaluate proposals for award. Evaluation factors other than price, when combined, are approximately equal to price in the award decision. All technical factors are relatively equal in weight. Information should be provided for the general contractor and major subcontractors. I. TECHNICAL A. Plan of Work: The government will evaluate the offeror's plan for accomplishing the work, including equipment, on-the-ground labor, subcontractors, and project supervision. If subcontractors are used for labor, higher score will be given to firms using reliable subs with whom they have an already-established working relationship. Lower scores will be given to firms who intend to subcontract to firms never used before, or do not have firm plans in place to use a specific subcontractor. B. Experience: particularly prior experience of the offeror and any subcontractor in direct performance of road construction, culvert assembly and installation, or similar civil engineering work. Direct experience in civil engineering work is preferable to construction management of subcontractors. II. PAST PERFORMANCE In the case of a newly-established offeror without a record of relevant past performance, the evaluation will take into account information regarding predecessor companies, key personnel who have relevant experience, or subcontractors who will perform significant portions of the requirement. When no information is available, the offeror may not be evaluated favorably or unfavorably. III. PRICE AWARD DECISION Award will be made to the most highly ranked offeror (1) whose total proposal is technically acceptable, (2) has demonstrated past performance in meeting customer satisfaction, and (3) whose past performance and technical/cost relationship presents the BEST VALUE and is the most advantageous to the Government. Evaluation factors other than price, when combined, are approximately equal to price in the award decision. The critical factor in making any cost/technical trade-off is not the spread between the technical scores but, rather, the significance of that difference. The significance of the spread of scores will be determined on the basis of what the difference might mean in terms of performance and what it would cost the Government to take advantage of it. Award may not necessarily be made for technical capabilities that would appear to exceed those needed for the successful performance of the work. The Government reserves the right to make cost/technical trade-offs that are in the best interest and advantageous to the Government.
