No proposals or quotes are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS or QUOTES and NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall NOT be construed as solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this notice is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Network Contracting Office (NCO) 16 is seeking preferably Service Disabled Veteran Owned Small Businesses or Small Businesses (but will accept other than small business for market research purposes), capable of providing two Cardiohelp Systems (heart lung support system) brand name only, for the Operating Room at the Michael E. DeBakey VA Medical Center located at 2002 Holcombe Blvd., Houston, TX, 77030-4298. See DRAFT Statement of Work for detailed description of Supplies. The NAICS code is 334510  Electromedical and Electrotherapeutic Apparatus Manufacturing and the Size Standard is 1,250 Employees. DeBakey VA Medical Center (MEDVAMC) currently owns an Extracorporeal Membrane Oxygenation (ECMO) machine (CardioHelp) and needs to replenishment of the disposable equipment supply, BEQ_HLS 7050 USA HLS Set Advanced 7.0 to support this machine. This supply will be used mainly by the Operating Room, SICU, and Cath Lab, but is not limited to these areas. Project Scope: The facility would like to stock two (2) units. LINE ITEM QUANITY UNIT OF ISSUE BEQ-HLS 7050 USA HLS Set Advanced 7.0 2 each Supply Specifications Requirements: Physical Characteristics (Brand Name Only) Integrated gas exchanger with diffusion membrane Integrated heat exchanger Integrated cutting edge VAD (Ventricular Assist Device) centrifugal pump Approval for 30 days and transportation approval Bioline coating (See below pictures.) Warranty and Support: N/A Delivery Location: Michael E. DeBakey VA Medical Center 2002 Holcombe Boulevard Houston, TX 77030 Installation: N/A Training: N/A Operations and Storage Areas: OR Protection of Existing Structures, Equipment, Utilities, and Improvements: The Contractor shall preserve and protect all structures and equipment on or adjacent to the work site. The Contractor shall replace at his own expense damage to such items to the satisfaction of the Contracting Officer. Contractor shall take all measures and provide all materials necessary for protecting and preserving existing equipment and property in affected areas of installation against dust, debris and physical damage, so that equipment and affected areas to be used in MEDVAMC operations will not be hindered. Contractor shall permit access to VA personnel through installation areas as required for maintenance and normal MEDVAMC operations. When the installation area is turned over to Contractor, Contractor shall accept entire responsibility therefore. Contractor shall maintain in operating condition, existing fire protection, alarm equipment and other operating equipment in the installation area. IT IS VERY IMPORTANT THAT ESSENTIAL AND LIFE SAFETY SYSTEMS BE CONTINUOUSLY MAINTAINED AND NOT INTERRUPTED WITHOUT TWO WEEKS PRIOR WRITTEN NOTICE AND APPROVAL FROM THE VA MEDICAL CENTER. Sign-in Procedures: All Contractor workers are required to sign in and out at the VA Police Dispatch at the corresponding locations as directed by the COR, or designee at each facility. A valid state driver s license or state identification card is mandatory for all employees to have access to these facilities. All contractor employees are required to wear the assigned VA badge at all times. If after-hour key service is needed, contact VA Police Dispatch at 713-791-1414, X7106. Material Safety Data Sheet (MSDS): N/A Work Hours: N/A ID Badges: N/A Estimates: Vendors will submit one quotation for the purchase of two units and match the requirements outlined in this Sources Sought Notice. In the quotes, vendors will indicate whether their solution has MET, NOT MET or MET WITH QUALIFICATIONS for each line item in this document (MET WITH QUALIFICATIONS will require a detailed listing of non-compliance). In addition, vendors will complete the Instructions to Offertory s and respond in detail to each question. Vendors are required to respond to each item individually in both documents.
