STATEMENT OF WORK NOTICE OF INTENT TO SOLE SOURCE: Naval Surface Warfare Center Indian Head Explosive Ordnance Disposal Technology Division (NSWC IHEODTD), intends to award a firm fixed price sole source procurement with North Star Imaging 19875 S Diamond Lake Road Rogers, MN 55374 To provide: Preventative Maintenance to include the following: Preventive Maintenance Agreement All INCLUSIVE AGREEMENT PERIOD: 7/1/18 - 6/30/19 System/Equipment: NSI X-View X5000 w/FXT-225.48-3 & HPX-450-11 Serial Number or NSI Job Number: 1304887 NSI Part #: PM12MCUS & SW12MCT North Star Imaging Inc. engineers, manufactures, integrates and tests equipment to ensure years of dependable operation. As with any precision equipment, X-ray systems need regular maintenance to ensure that the X-ray set remains free of contamination and to monitor and replace wearable components when needed. Purchasing a preventive maintenance agreement will prevent unplanned downtime due to unnecessary failures, extend the life of your system, maximize equipment performance and minimize maintenance costs. North Star Imaging preventive maintenance programs include a 12 Point Inspection performed on a bi-annual basis by factory trained technicians and authorized imaging specialists. Our goal is to protect your investment. 12 Point Inspection System Maintenance Includes: • Clean, inspect X-Ray Tubes, replace O-rings and adjust to manufacturers specs. • Clean, inspect, reapply dielectric grease to HV cables and set compression. • Clean, inspect and lubricate manipulator. • Clean cooler and test flow and temperature switches. • Clean and verify adjustments on the HT generators to prolong tube filament life. • Test and adjust Safety Interlocks and Safety Lamps. • Test power supplies and adjust to factory specifications. Preventive Maintenance Agreement All INCLUSIVE • Test and adjust shutter. • Service vacuum system. • Inspect for proper cable drape. • Verify emergency stop operation • Perform a Radiation Safety Survey with documentation. Upon completion of the 12 Point Inspection and Preventive Maintenance the assigned technician/imaging specialist will provide documentation of findings and recommendations. PREVENTIVE MAINTENANCE AGREEMENT CUSTOMERS RECEIVE THE FOLLOWING BENEFITS: PREFERRED SERVICE PM Agreement customers will receive preferential service scheduling over standard repair service requests. DISCOUNT ON SERVICE LABOR Non-scheduled emergency service and scheduled onsite service visits performed by NSI will be billed at $hr in lieu of the standard labor rate of $/hr. Travel labor will be billed at $/hr. Labor prices will not increase during the agreement period. DISCOUNT ON SERVICE PARTS Excludes X-Ray Tubes, and Imaging devices. Replacement parts carry a 90-day warranty. UNLIMITED TELEPHONE AND REMOTE TECHNICAL SUPPORT NSI technical staff are available Monday - Friday, 8:00 AM - 5:00 PM (CST) for telephone and remote access support. On-call service staff are available 24/7 via voicemail (763-463-5658) or support email (support@4nsi.com). Overnight/Weekend/Holiday calls will be returned within 24 business hours. SOFTWARE MAINTENANCE Unlimited Software Updates (Download Link) A software agreement is less expensive than purchasing software updates, upgrades and support separately. During the term of your agreement, you will receive unlimited software update via download link within the one-year agreement period. The software updates include service packs at no charge for all major and minor updates. Preventive Maintenance Agreement All INCLUSIVE ADDITIONAL ALL-INCLUSIVE CONTRACT BENEFITS All INCLUSIVE PREVENTIVE MAINTENANCE AGREEMENT CUSTOMER RECEIVE THE FOLLOWING BENEFITS: Additional Service Trips for Unexpected Down Time Including Parts and Labor •• • Two Additional Service Trips for Unexpected Down Time Guidelines to the Additional Service Trips: If the technician onsite is unable to diagnose or fix the issue and needs to leave, another trip will be provided with no penalty against the trips provided. If the technician onsite can diagnose and fix the issue prior to leaving and the issue persists after their departure where a return trip is requested, that will count against the trips provided. Discount on NSI Training Services All-Inclusive Preventive Maintenance Contract customers receive discount on all NSI Training services. •a. Pricing for Basic Year: 07/01/2018-06/30/2019 •b.
