- This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 36C25018Q9355 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99, dated 15 June 2018. The associated NAICS Code is 337214, Office Furniture (except Wood) Manufacturing, and the Small Business Size Standard is 1,000 employees. This is a 100% total Service Disabled Veteran Owned Small Business (SDVOSB) set-aside procurement. REQUIREMENTS: MFG: MIDMARK (or Equal ) Item Description/Part Number* Qty Price 6283-001 Heavy Duty Wall-Mounted Workstation 20 9A593001 Dual Monitor Arm 20 9A59001 Tamper Proof CPU Holder 20 SALIENT CHARACTERISTICS: Monitor shall have at least 9.5 of height adjustment, 45 degrees of tilt and 180 degrees of independent rotation from the keyboard for proper positioning. Keyboard shall have 22 of height adjustment for proper ergonomics. Shall have upper level work surface for papers and small technology above the keyboard for staff s educational documentation and script pad. Wall arm shall have up to 180 degrees of rotation with a pivot limitor to lock monitor in place every 90 degrees. Shall be able to fold up 9.47" off the wall when the tray is flipped up without touching the wall for space optimization. Shall have integrated wire management system in the mounting arm for neat and quick wire installation. Wall track shall be available in at least 34" length for direct to drywall applications to optimize locations within PACT exam room. Wall track shall use proven suitable Heavy-Duty Hollow-Wall anchors for mounting. Shall have dual monitor system that can support (2) 24" monitors and 24lbs to properly view and display patient history and educational material. Shall comply with TAA and with RoHS regulations. Shall have a weight capacity of 20-35lbs for proper support. Shall have tamper proof CPU holder attached to the wall track for proper cord management that is approximately 15.74" W x 12.13" H x 3.96" D. Shall have the option to add a 20" extension arm to properly position the monitor in the room. Shall be able to adjust tension and tilt on the wall arm with Allen wrench. At minimum, the offered item must meet all product specifications/salient characteristics of the requested item to be considered a responsive offer. Set-Aside Requirement: Only verified SDVOSB and VOSB firms are eligible to submit an offer or receive an award of a VA contract that is set-aside for SDVOSBs or VOSBs in accordance with VAAR Part 819. A non-verified vendor that submits a bid or proposal should be rejected as non-responsive or technically unacceptable as listing in VIP is not self-correctable as a minor informality as, for example, a listing in SAM. All other purported SDVOSBs and VOSBs must apply for and receive verified status in accordance with 38 CFR Part 74 and be listed in VIP prior to submitting a bid or offer on an acquisition conducted in accordance with VAAR Part 819. The VIP database will be checked both upon receipt of an offer and prior to award. New Equipment ONLY; NO remanufactured or "GRAY MARKET" items. All items must be covered by the manufacturer's warranty. No telephone requests for information will be accepted. Only emailed requests received directly from the Offeror are acceptable. It is the Offeror s responsibility to ensure the quote is received. If an equal item is offered, the offeror must submit descriptive literature that conclusively establishes product equivalency and technical acceptability and compatibility at time of quote submission. IF THE OFFEROR IS NOT THE MANUFACTURER OF THE QUOTED ITEMS, THE OFFEROR SHALL SUBMIT WRITTEN EVIDENCE AT THE TIME OF QUOTE SUBMISSION THAT CLEARLY VERIFIES THEY ARE AN AUTHORIZED DISTRIBUTOR AND/OR RESELLER OF THE QUOTED PRODUCTS and will provide the items on or before the specified delivery date. EVALUATION CRITERIA/SUBMISSION OF OFFER: Evaluation: This is a competitive RFQ under the authority of FAR 13, Simplified Acquisition Procedures. The Government will issue a Firm-Fixed Price Purchase Order resulting from this solicitation to the responsible Contractor whose quote conforms to the requirements within this solicitation and will be the most advantageous to the Government, price and other factors considered.
