Description
SOURCES SOUGHT FOR HCHV CERS POPLAR BLUFF VAMC Purpose: The purpose of this Sources Sought Notice is to provide notice of the forthcoming solicitation for Healthcare for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) for the Department of Veterans Affairs Poplar Bluff VA Medical Center (VAMC). It is to collect and analyze information regarding potential sources for the services required and source capabilities within the services to homeless persons market, to satisfy the agency s needs and enhance competition. This is not a solicitation for work, nor is it a request for a price offer/proposal/quote. Background: The HCHV Program at the Poplar Bluff VAMC is an essential and critical part of their function to serve veterans and their health care needs in the Poplar Bluff VAMC. Through the HCHV program, the VA provides outreach and case management services to veterans, facilitating their access to a broad range of medical, mental, health, and rehabilitative services, in conjunction with work therapy. Emergency housing is to be a transitional environment that is a supportive atmosphere for the veteran who may suffer from physiological illness, psychological illness, and/or substance abuse and who currently are living "On-The Streets, in shelters or experiencing housing instabilities. General Description of Services: The VAMC requires one or more contracted facilities to provide homeless veteran services at one or more facilities located within the surrounding geographical area of the John J. Pershing VA Medical Center, 1500 N. Westwood Blvd. Poplar Bluff, MO 63901. This procurement may be a single or multiple award based on several factors - facility capacities - abilities to securely house both male and female veterans within the same facility - abilities/limitations regarding housing either male-only or female-only location ability to provide recurring transportation to/from Poplar Bluff VAMC and other related appointments manage medication storage provide mailing address for veterans. The procurement type (contract type) will be for services, non-personal, Firmed Fixed Price Indefinite Delivery Indefinite Quantity (FFP IDIQ) contract(s). Services required (capacity) is expected to be approximately 22 per any given day (regardless of number of contractors), however "spikes" in usage should be considered. Estimated veteran consensus, based on recent placement history, is about 90-92% male and 8-10% female. The VA requires pricing to be "Per Bed Days of Care" (BDOC) - meaning - One (1) veteran placement for 1 day equals 1 BDOC. 1 BDOCs will be inclusive of safe housing (with bedding, linens), hygiene items and laundry capabilities/facilities, telephone and computer access, medications management (locked storage), 3 meals and nutritious snacks, community outreach, transportation to/from VA Appointments, structured periodic groups and activities and other requirements. Any FFP IDIQs awarded will include a Base Year (1 calendar year) Period of Performance. Four (4) option years will be available for additional performance periods at the sole discretion of the Government. Objective of RFI/Sources Sought Notice: To receive marketplace feedback and capabilities or limitations regarding the required services: Please provide information as applicable to your facility and services capabilities, or planned capabilities. The items regarding Facility, Nutrition and Food, Clinical Review, Physical Security, Medication, and Staffing are included in VA HCHV facility Inspections. Compliances with codes and laws must be evidenced by appropriate inspections/permits. Facility information Nutrition and Food Clinical Review Physical Security Medication Staffing Place of Performance: Contracts will be awarded to offerors having an acceptable facility that passes VA HCHV Inspection Requirements, and be within the Poplar Bluff surrounding geographic area suitable for recurring transportation to and from the Poplar Bluff VAMC. If no acceptable HCHV facilities are available within this area, consideration will be given to the Contractor whose facility meets all the stated requirements and is closest to the Poplar Bluff VA Medical Center. Milestone Objective: The VA anticipates issuing a solicitation for this requirement during the month of July 2018. This notice is issued for 15 Calendar Days Beginning June 30, 2018. Responses are due by July 16, 2018, 4:30 pm (CDT/DST). The resulting solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation, and any attachment, including any subsequent amendments to the solicitation or updated attachments from that website, and no hard copies will be made available. The solicitation will provide full information to interested parties about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation, attachments and any amendments. NAICS Code: is 624221 with a small business size standard of $11 million. Based on the Contracting Officers completed Market Research report IAW Far part 10, will depend on how the Solicitation will be issued as full and open competition, or will include Limited Sources. All vendors must be registered in the System for Awards Management database(www.sam.gov) to be eligible for award of a federal contract, and have input Vet Biz information at http://www.vip.vetbiz.gov if a veteran-owned business. Registration in SAM is free and potential offerors are encouraged to visit the website and register as soon as possible to avoid delays should they be selected for a contract award.
