Attach / upload the "Questions and Answers" submitted in response to the solicitation. 2. Attach / upload Amendment 0004 of Solicitation Number 1605DC-18-R-00020, which incorporates all changes related to the Questions and Answers document. Amendment 0003 Combined Synopsis / Solicitation Pharmacy Benefits Management (PBM) Services Department of Labor (DOL) Office of Workers' Compensation Programs (OWCP) Solicitation Number 1605DC-18-R-00020 The purpose of Amendment 0003 of Solicitation Number 1605DC-18-R-00020 is to: 1. Re-open Solicitation Number 1605DC-18-R-00020 and establish a new "Questions and Answers" period, and a new Due Date for the submission of proposals. A. The new Due Date for questions and comments concerning the solicitation is MONDAY, JUNE 18, 2018, 8:30 AM EST. SPECIAL NOTE REGARDING QUESTIONS: Offerors are instructed to carefully review the "Questions and Answers" document posted with Amendment 0001 of Solicitation Number 1605DC-18-R-00020 before submitting questions in response to the re-opened solicitation. B. The new Due Date for proposal submission to MONDAY, JULY 09, 2018, 8:30 AM EST. SPECIAL NOTE REGARDING PROPOSAL SUBMISSION: Offerors who submitted timely proposals by the previous Due Date of FRIDAY, MAY 25, 2018, 8:30 AM EST are encouraged to submit new proposals due to nature and extent of the new information provided via the solicitation re-opening. However, Offerors who previously submitted a proposal by the FRIDAY, MAY 25, 2018, 8:30 AM EST Due Date ARE NOT REQUIRED to re-submit a new proposal. The submission of new proposal information is at the discretion of the Offeror. Additionally, Offerors who previously submitted a proposal ARE NOT REQUIRED to re-submit Past Performance Questionnaires received by the Government. 2. Provide an updated Claimant Eligibility File layout. 3. Provide updated Pharmacy Transaction Data including National Drug Code (NDC) and National Provider Identifier (NPI) information. 4. Extend the due date for proposal submission to FRIDAY, MAY 25, 2018, 8:30 AM EST. Combined Synopsis / Solicitation Pharmacy Benefits Management (PBM) Services Department of Labor (DOL) Office of Workers' Compensation Programs (OWCP) Solicitation Number 1605DC-18-R-00020 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and proposals are being requested. The combined synopsis/solicitation information is as follows: (1.) Solicitation Number: 1605DC-18-R-00020 This solicitation is issued as a Request for Proposal (RFP). (2.) The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97, dated January 24, 2018. (3.) The requirement shall be posted under the North American Industry Classification System (NAICS) Code of 524292 - Third Party Administration of Insurance and Pension Funds. The Small Business Size Standard for NAICS Code 524292 is $32.5 Million. The requirement for Pharmacy Benefits Management (PBM) Services shall be competed utilizing "Full and Open Competition" with no Set-Asides. (4.) Required Services: Information related to the Contract Line Item Numbers (CLINs), Quantity, and Unit of Measure is contained in the attached Solicitation Number 1605DC-18-R-00020. (5.) Description of requirements for the items to be acquired: The contractor shall implement and operate a PBM in the United States and its territories for the Division of Federal Employees' Compensation Programs (DFEC) that injured federal employees will generally be required to use in order to access pharmacy benefits. The PBM includes a program that services Federal employees with work-related injuries or illness, that have an accepted workers' compensation case with DFEC, and require prescription medications, diagnostic services (e.g., magnetic resonance imaging (MRI), computerized tomography, (CT), radiography, electromyography (EMG)), physical therapy (PT), and durable medical equipment (DME). The contractor shall provide a system for first-fill prescription coverage for employees who sustain a traumatic injury at work. The contractor shall establish a formulary for approval by OWCP to better manage costs, improve safety, and implement drug utilization review processes. For additional information, please refer to the attached Solicitation Number 1605DC-18-R-00020. (6.) Date(s) and place(s) of delivery: The anticipated Period of Performance shall be for one (1) twelve (12) month Base Period and four (4) 12-month Option Periods. The Government anticipates that work shall be performed off-site at the Contractor's facility or field offices. (7.) The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Specific information related to the Addenda to FAR 52.212-1 is contained in the attached Solicitation Number 1605DC-18-R-00020. (8.) The evaluation procedures to be used are contained in the attached Solicitation Number 1605DC-18-R-00020. (9.) All Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. FAR 52.212-3 can be accessed electronically at the following web address: https://www.acquisition.gov/ (10.) FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, shall apply to this acquisition by reference-no addenda shall be attached. (11.) FAR Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, shall apply to this acquisition. The specific FAR Clauses selected are contained in the attached Solicitation Number 1605DC-18-R-00020. (12.) Additional Contract Requirements: Additional clauses included by reference and in full text are contained in the attached Solicitation Number 1605DC-18-R-00020. (13.) Defense Priorities and Allocations System (DPAS) and assigned rating: N/A. (14.) Due Date for Proposals: It is requested that you submit a proposal or response by Friday, Friday, May 11, 2018, no later than 1:00 PM EST.
