- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) - Southern Tier in North Chicago, IL is soliciting businesses for Removal and replacement of Battery backup lamps in surgical suite at the Jesse Brown VA hospital located in Chicago, IL. The current systems and devices are listed in the attached SOW. It is understood that all Contractors submitting a response to this Sources Sought will have an established business with three (3) years of consecutive experience in performing these types of services at healthcare facilities. The Contractor must also be licensed and authorized to perform services on all the equipment listed. The Contractor shall furnish all labor, supervision, materials, tools, and equipment necessary to complete the required services. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 238210 Electrical equipment and appliance installation with a Size Standard of $15 Million in annual revenue. Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. Capability statements shall also include: DUNS number Organization Name and Assumed Name/dba (if applicable) Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size (small business or other than small business) Socio-economic status (If SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Organization experience in performing these types of services Copies of applicable certifications and/or accreditations (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 238210. If another NAICS code may be applicable, please identify the NAICS in your capability statement along with your corresponding socio-economic status. The Government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated. SERVICE STATEMENT OF WORK FOR Place of Performance: Bed Pavilion Surgical Suites Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Part 1: Project Scope of Work Overview: Contractor shall provide all tools, materials, to replace (53)- surgical grade 6-lamp T5 battery back-up 2 x 4 fluorescent light fixtures with 90-minute battery power pack. Part 2: Period of Performance (POP) and Project Construction Cost Magnitude: Complete all work including submittals, reviews and quality control within 7 calendar days from date of award. Work to be performed during surgical blackout dates TBA. Part 3: Detailed Project Scope of Work: The contractor shall provide the following: Task 1 Contractor shall schedule meeting with COR to collect information on existing fluorescent and canned battery backup lighting specifications and intelligent dimmer controls. Task 2 Contractor shall provide and install (53) new 2 x 4 6-lamp T5 surgical grade fluorescent light fixtures made to the same specifications of existing surgical grade 2 x 4 6-lamp T5 battery 90-minute battery backup lighting. Task 3 Contractor will install, wire, program, upgrade devices and certify that new battery backup fluorescent light fixtures will function and operate properly with the existing Leviton Synergy dimmer controlled lighting system and all correlating operatory lighting. Task 4 Contractor will confirm that battery back lighting will perform to the full 90-minutes when utility power is shut off. Construction Progress Meetings: The Contractor s Project Manager shall provide daily progress meetings with the COR for the entire period of performance. Include minutes of the meeting and issue past minutes not later than 1 days after the meeting. Construction Progress Meetings Minutes shall contain: list of attendees, list any issues or concerns, indicate days remaining per the contract, detail any safety incidents, RFI log, detailed 2 week look ahead and shutdown schedule to start after date of award Part 5: Applicable Performance Standards All work shall comply with all VA Policies including and not limited to: VA Construction Safety and VA Infection Control Policies. The Prime Contractor shall have a Competent Supervisory Person on site at when any worker(s) or sub-contractors are present. All persons working on any electrical systems rated 50 volts or more shall have the minimum qualification as a State Licensed Master Electrician or Registered Journeyman Electrician, all activities shall follow the safe work practices in compliance with NFPA 70E Electrical Safety in the Workplace. The Contractor shall provide mitigating activities: Fire extinguishers, pedestal mounted in the area of work No trash, used packaging or construction spoils shall be stored on site. Remove each day using covered gondolas. Broom sweep the work area at the end of each day. All work will be provided with 1 year parts and labor guarantee from date of acceptance by the VA. Part 6: Jesse Brown VAMC Additional Requirements Comply with all Jesse Brown VAMC site specific requirements and SOPs including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. Normal construction work hours are 7:00 am to 3:30 pm Monday through Friday. Normal work hours can be modified only by written approval of the COR. No work by the Contractor on Federal Holidays and no work on the weekend when the Federal Holiday is on the respective Monday or Friday. The Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is in Patient Admitting for any work between 6:00 pm through midnight to 6:00 am or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD.
