Description

SF30 Amendment #5 AMENDMENT NO. 5 The purpose of this Amendment No. 5 is to clarify Past Performance Questionnaire, clarify how price will be evaluated, remove the requirement for a color additive to the asphalt, and extend the proposal due date. a. Request for Proposal (RFP), Amendment No. 4, reference the Past Performance Questionnaire, all forms received prior to the proposal due date or with the contractor's proposal will be considered. b. RFP, Section M - Evaluation Factors for Award, Volume II, Non-Technical Factor - Price, delete the second paragraph. A detailed breakdown is not required with contractor's proposal. c. RFP, Amendment No. 2, Summary of Items Amended, Item #5, the requirement for a color additive to the asphalt is removed. d. Proposal due date and time is changed from not-later-than "July 3, 2018, by 2:00 pm MDT" to "July 6, 2018, by 2:00 pm MDT".
