Description
This is a REQUEST FOR INFORMATION (RFI) only. Market research is being conducted to determine the amount of qualified and capable businesses that can provide the items requested below. If you believe your firm is able to provide the items requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. Due date for responses is close of business (4:30PM local time/CT) on Tuesday, July 17, 2018. Email responses to Melanie Stockman at melanie.stockman@va.gov. All questions must be submitted in writing to the Contracting Officer in writing via email. Telephone inquiries will NOT be accepted/returned. When submitting a response to this RFI, offerors must include the following: Indicate socio-economic status Include picture of offered items and support literature, brochure etc Indicate any existing Government contract where item(s) may be purchased (i.e. FSS/GSA contract etc) Must be able to provide delivery to all sites and training for biomed staff DISCLAIMER-This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201 (e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. NAICS: 339112 Size Standard: 1000 employees SALIENT CHARACTERISTICS: Flexion/distraction table suitable for chiropractic exam and treatment with the following features: Foot Controls: Elevation Lowest height 20 or shorter Highest Height 30 inches or taller Tension at least two settings Color to be determined by interior designers Manual drops Pelvic Thoracic Lumbar T-Bar Handle - short Auto Flexion, Long Axis distraction, circumduction Flexion at least 15 degrees extension at least 10 degrees side bend at least 30 degrees Break-away Chest with dual side control Cervical Tilt, Elevation, Forward & Toggle Drop with Adjustable Cushion Cervical distraction Adjustable, tilt-table Face cushion At a minimum, table must be able to sustain 350-pound patient weight Cushion minimum 22 wide Table length not to exceed 80 inches Expanding cover to hide mechanisms Ankle rest with cuffs for safety Quantity 8: Delivery: Delivery is expected as soon as possible after contract is complete. 4 Tables will be delivered to 4101 Woolworth Ave Omaha, NE 68105 2 Tables will be delivered to 600 South 70th Street Lincoln Ave, 68510 2 Tables will be delivered to 2201 North Broadwell Avenue Grand Island, NE 68803 For each table: Vendor will supply: User manual Parts list Assembly and installation instructions. Care and Cleaning instructions Additionally, 1 extra copy of the above documents will be included, free of charge, and provided to the Omaha Biomedical Department. Vendor will provide Biomedical Service Technician training for 2 technicians and will convey all access needed to service the equipment. Date of manufacture must not be greater than 2 years before the purchase order date. The equipment must not be used or refurbished and must have the latest manufacturer s software/hardware updates installed.     Any batteries provided with the unit will not be manufactured more than 12 months before the purchase order issue date.
