Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is 36C24518Q9152 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) The North American Industry Classification System (NAICS) code is 334510, and the size standard is 1,250 Employees. (v) This requirement consists of six (6) line items: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT 0001 EV1000 CLINICAL PLATORM NI (KIT CONTAIN EV1000A CLINICAL PLATFORM AND NI CLEARSIGHT MODELS (EVPMP, PC2K, EVHRS, EVE3FT, EVNIDBAC, EVNIPCL3, EVPMAC) 6.00 EA 0002 CABLE, FLOTRAC, 16 FT LOCAL STOCK NUMBER: EVFTCL 8.00 EA 0003 ROLL STAND LOCAL STOCK NUMBER: EVRS 8.00 EA 0004 PRESSURE CONTROLLER COVER PCCVR Included with EV1000CS / EVNIUPG-US kits 8.00 EA 0005 EVNITPSPM12 Service Agreement with Preventive Maintenance covers the following models: EV1000A, EVPMP, PC2K and EVHRS 8.00 EA 0006 EQUIPMENT TRADE-IN - CONTRACTOR IS RESPONSIBLE FOR ALL SHIPPING COSTS ASSOCIATED WITH RETURNING THESE ASSETS TO EDWARDS LIFESCIENCES. CONTRACTOR WILL HAVE 30 DAYS FROM DATE OF DELIVERY OF ORDER TO REMOVE ITEMS FROM THE MEDICAL CENTER. 2 Vigileo for 2 EV1000 2.00 EA (vi) To be technically acceptable the products quoted shall be Brand Name to the items noted above. (vii) Delivery FOB Destination to VA Medical Center, 10 North Greene Street, Baltimore, MD 21201. (viii) 52.212-1 Instructions to Offerors - commercial Items, applies to this acquisition. There is no addenda to the provision. (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- the offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) Website through http://www.sam.gov/portal. If the offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (p) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: applies to this acquisition.
