- STATEMENT OF NEED BACKGROUND The Department of Veterans Affairs Erie Pennsylvania for the Ashtabula County (Ohio) Community Based Outpatient Clinic and the Crawford Country (Pennsylvania) Community Based Outpatient Clinic has a brand name or equal requirement to purchase 1 each (2 total) Midmark 630 Human Form Power Procedure Tables to include two chair arms for the new Podiatry services that will be provided at these clinics. These chairs will allow for proper medical positioning to ensure proper access to the patient. The size and height of the procedure chair is such that the patient can enter and exit the chair in a safe manner for themselves and the clinical staff. This chair will also be utilized for other medical services at a later date such as Optometry. DELIVERY AND SHIPPMENT: All items quoted must be F.O.B. destination. The vendor will deliver the equipment within 15 days of award between the hours of 8am and 3:30pm Monday through Friday. All items will be shipped to the following location Erie VA Medical Center - Warehouse 135 East 38th Street Erie, PA 16504 INSTALLATION AND TRAINING Installation and Training will not be required by Vendor.
