Page 1 of 2 The Department of Veteran Affairs, Network Contracting Office 2 (NCO 2), intends to issue solicitation # 36C24218R0522, in order to reissue the requirement previously solicited on cancelled solicitation # 36C24218R0248, for Prescription Eyeglasses and Optician Services, for multiple VA locations within the VISN 2 Downstate geographic area. The following VA VISN 2 Downstate locations will be included in the resultant contract: VA New York Harbor Healthcare System Manhattan Campus 423 East 23rd Street New York, NY 10010 VA New York Harbor Healthcare System Brooklyn Campus 800 Poly Place Brooklyn, NY 11209 VA New York Harbor Healthcare System St. Albans Campus 179th & Linden Blvd St. Albans, NY 11425 VA Hudson Valley Healthcare System, Castle Point 41 Castle Point Road Wappingers Falls, NY 12590 VA Hudson Valley Healthcare System, Montrose 2094 Albany Post Road Montrose, NY 10548 VA New Jersey Healthcare System, East Orange 385 Tremont Avenue East Orange, NJ 07018 VA New Jersey Healthcare System, Lyons 151 Knollcraft Road Lyons, NY 07939 James J. Peters VA Medical Center Bronx 130 West Kingsbridge Road Bronx, NY 10468 Northport VA Medical Center 79 Middleville Road Northport, NY 11768 In accordance with the Statement of Work, the Contractor shall furnish prescription eyeglasses, to include lenses and frames, and onsite (at VA medical centers) optician services to eligible veterans. The Contractor is to provide all labor, materials and transportation necessary to provide prescription eyeglasses according to contract requirements for beneficiaries of the VA Healthcare Network Downstate New York (VISN 2). All items provided by the contractor shall be new. The contractor shall provide completely assembled eyeglass frames placed in standard alignment and include mounting of lenses. All items provided by the contractor shall include fitting and adjustment services and a minimum of one year warranty. The resultant contract will be a single award, Indefinite Delivery Requirements type contract on a firm fixed price (FFP) basis, for a base period of one (1) base year, plus four (4) one year option periods. The solicitation will be issued at the Federal Business Opportunities (FBO) website (https://www.fbo.gov) on/about July 25, 2018. The North American Industry Classification System (NAICS) Code is 339115 Ophthalmic Goods Manufacturing, Small Business Size Standard 1000 Employees. Contract award will be based upon the tradeoff process in accordance with Federal Acquisition Regulation (FAR) Subpart 15.101-1. This procurement is a total Service Disabled Veteran Owned Small Business set aside. In order to be considered for award, prospective Offerors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM prior to award of a contract. SAM is located at http://www.sam.gov and consolidated the capabilities of CCR/Fed Reg., ORCA, and EPLS. There is NO cost to use SAM. This notice is for informational purposes only and is not a request for submission of proposals. No specific response to this pre-solicitation notice is required at this time. No potential offeror list is maintained. All potential offerors should register with Federal Business Opportunities if interested in this acquisition. No additional information is available other than what is noted herein.
