- FAR 52.212-5 Commercial Items, Jan 2018. Solicitation Clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2018-25570 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention/NIOSH-Cincinnati/DSHEFS intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure: CLIN 0001: P/N 220-5000TC-K5D, SKC AirChek TOUCH 5-pack High/Low Flow Pump Kit includes 5 pumps with Li-Ion battery packs, 5 screwdriver sets, 4 Standard Charging Cradles, 1 Enhanced Charging Cradle (e-Cradle), power supply with cord, 5 each: filter cassette holders, All-in-One adjustable tube holders, and Type A Protective tube covers, in a Pelican case; 100-240V. Price each Total Price Quantity: 8 each -- 5-pack (total 40 pcs) $_________ $_________ Grand Total $_________ The government intends to award to the lowest priced technically acceptable offer (LPTA). Your quote shall include the following: • Total Price for CLIN 0001 and Grand Total • Vendor name, address and DUNs Number. Requested Delivery Date: 6 weeks ARO, vendor to indicate lead time, ___________. Delivery Address: CDC\NIOSH\DSHEFS-Cincinnati 1090 Tusculum Cincinnati, Oh 45226-1938. FOB point: Origin. Freight will be paid by requesting Program's Shipping Account. The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-98 (05/01/2018). This acquisition is under North American Industry Classification System (NAICS) code 334519 and small business size standard 500/employees. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
