Attach D- Quality Assurance Surveillance Plan (QASP) Attach C- Provisions and Clauses Attach B - Statement of Work Attach A - Pricing Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal (RFP) and the reference number is RFP-18-PHX-06. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98 and the Health and Human Services Acquisition Regulation (HHSAR). The Offeror(s) must quote on an all or none basis. Written quotes ARE required (oral offers will not be accepted). Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Item - Evaluation FAR 52.212-2. The Offeror shall complete 52.212-3 Offeror Representations and Certifications - Commercial Items, paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision. The basis for evaluation shall be on best value with tradeoffs. Offerors shall provide a three (3) to twelve (12) page Technical solution to the Performance Work Statement and a minimum of three (3) references who can validate recent and relevant past performance. Solicitation is issued using Simplified Acquisition Procedures in accordance with FAR 13.106-2 and FAR 13.106-3. Clauses can be obtained in full text at: http://farsite.hill.af.mil. Additional information required: CAGE Code, DUNS, Tax Identification Number, FOB Point, Discount Terms and Delivery Date. This procurement is unrestricted under associated NAICS Code 621399 (Office of All Other Miscellaneous Health Practitioners) with Size Standard of $7.5 Million. This is a requirement of obtaining Dietician Services for Desert Visions located in Sacaton, Arizona and Nevada Skies located in Wadsworth, Nevada. Special Notes: Best Value Determination. The best value with tradeoffs quote will be determined to represent the best value. Technical acceptability will be determined by an evaluation of the quoted requirements that meet the technical criteria that is contained in the solicited item descriptions. The tradeoff method is used when it is in the Government's best interest to consider award to other than lowest priced offeror, or to other than the offeror given the highest ratings for technical, management, past performance, or other non-cost/price factors. Under the tradeoff approach, the Government evaluates both cost/price and other performance factors. Award is made to the offeror whose proposal offers the best value to the government, considering trade-offs between price/cost and the other factors. All evaluation factors and significant sub-factors that will affect contract award and their relative importance must be clearly stated in the solicitation. The solicitation must state whether all evaluation factors other than cost or price, when combined, are significantly more important than, approximately equal to, or significantly less important than cost or price (FAR 15.101-1).Award will be made to the responsible vendor whose quote is the determined to be the best value with tradeoffs. Site Visit. Not Applicable. Notice to Offeror. The Government reserves the right to cancel this purchase request, either before or after suspense date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. Discussions. The Government intends to award a purchase order without discussions with respective vendors/offerors. The Government however, reserves the right to conduct discussions if deemed in its best interest.
