COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for EXAM TABLES (IDIQ) BRAND NAME, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24418Q0593 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 337127 code has a small business size standard of 500. The Contractor is guaranteed a minimum order of (16) Midmark Exam Table units (Model#: MI-626-003) to be delivered to the requested VA Medical Centers and Clinics as stated. Background: The Lebanon VA Medical Center (LVAMC) has an estimated need for large amount of Midmark Exam Tables and Accessories over the next 5 years. Currently, the VA Medical Center is going through two VA Clinic Activations with more expected and will need to issue multiple delivery orders over the term for each facility. This IDIQ will fulfill this requirement and any upcoming needs to replace or add Midmark 626 Exam Tables in the Lebanon VA Medical Center and supporting medical clinics during the period of performance. Performance Period: POP: (7/24/2018 to 4/16/2023) or 5 Years from date of contract award. Orders will be requested by Delivery Order and shall occur within 30 days of receipt or according to the schedule outlined by the COR. The Midmark 626 Barrier Free Power Exam Table and Accessories will improve patient care since the exam table lowers to 18 off the floor for easy accessibility and the exam table calculates the weight of a patient while seated on the exam table. Midmark 626 Barrier-Free Power Exam Table (Model#: MI-626-003) w/ receptacles, pelvic tilt, drawer heater, digital scale, USB hub, wireless controls, One touch Auto return home button, programmable one touch quick exam button, multi-position hideaway stirrups, treatment pan, and spline receptacle system. Lincoln Ave., Lebanon, PA 17042 Cumberland County VA Clinic or 5070 Ritter Road, Mechanicsburg, PA 17055 Lancaster County VA Clinic or 212 Willow Valley Lakes Drive, Willow Street, PA 17584 York VA Clinic or 2251 Eastern Boulevard, York, PA 17402 Schuylkill VA Clinic or 1410 Laurel Boulevard, Pottsville, PA 17901 Berks County VA Clinic or 2762 Century Boulevard, Wyomissing, PA 19610 Shipping information and a tracking number shall be provided to the COR prior to delivery.
