- Attachment D - Statement of Work Attachment C - Tax Exemption Attachment B - BAA Attachment A - Rate Schedule RFP 18-247-SOL-00035 Contractor shall provide Emergency Room (ER) Physician Services at the Hopi Health Care Center, Polacca, Arizona on an as needed basis in accordance with the terms specified in the attached Scope of Work and contract terms and conditions. Period of Performance is to start on September 1, 2018 through August 31, 2019. Closing Date is August 3, 2018 at 1700 hours local time (AZ time). Deadline for questions is on July 27, 2018 at 1700 hours local time (AZ time). Contractor shall state in their proposal that the quote shall remain effective for 120 calendar days after close of solicitation. The Government reserves to right to make a single award, multiple awards or no awards in accordance with provision at FAR 52.215-1 which is included in this solicitation. Contractor is to follow and submit proposals in accordance to Section E Solicitation Provisions and Addendum to FAR 52.215-1 Instructions to Offerors - Competitive Acquisitions.
