- THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS, IN CONJUNCTION WITH FAR PART 13 SIMPLIFIED ACQUISITION PROCEDURES AND SUPLIMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-96, dated November 6, 2017. Solicitation 36C26218Q9493_1 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. In accordance with 38 USC 8127(d) Veterans First Contracting Program, and VAAR 852.215-70 Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors, this solicitation is an SDVOSB set-aside. NAICS Code is 322291 and the Small Business Administration s (SBA) size standard is 1500 Employees. Only offers from contractors listed as SDVOSB on the VETBIZ website will be considered. The Department of Veterans Affairs, Network 22 Contracting Office is seeking sources for 1440 cases of jumbo paper towel rolls for the VA Greater Los Angeles Healthcare System. This solicitation contains a brand name or equal justification for the following system components: Brand Name: Georgia Pacific THE PAPER TOWELS MUST MEET THE FOLLOWING CHARACTERISTICS Must be compatible with the Touch-Free EnMotion Dispenser Must be the Jumbo size Must be 1-ply, 10-inch X 800 foot Place of Performance: VA Greater Los Angeles Healthcare System (VAGLAHS), 11301 Wilshire Blvd., Los Angeles, CA 90073 Period of Performance: Delivery will be as required by the end user from 8/1/2018 7/31/2019. Provisions: 52.212-1 Instructions to Offerors Commercial and 52.212-2 Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm s DUNS#, total price and a completed copy of offeror s annual representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications Commercial Items.
