Page 1 of 2 COMBINED SYNOPSIS/SOLICITATION Digital Hybrid Myd/non-Myd Retinal Camera Buffalo VA Medical Center This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). RFQ reference number 36C24218Q9705. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. This solicitation is 100% set-aside for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSBs). The North American Industry Classification System (NAICS) code is 339113, with a small business size standard of 750 employees. This is an add on to what is already being used at the Buffalo VA. Canon USA - Digital Hybrid Myd/non-Myd Retinal Camera Description of requirement: The Buffalo VA Medical Center has a requirement for Digital Hybrid Myd/non-Myd Retinal Camera. AutoMosaic) 0008 01 each - 2465V347-K2 Table (20"x36") 0009 01 each - 2206V676-Hon-Task/ LabStool with Back 0010 01 set - 2748V535-MasterCaster 2" w/Lock set of 5 (For Task Stool with Back) Required Salient Characteristics Voltage Range: 100-240V ~ HP Pro-One 600 G2 (all in one PC) ~ Image capture plus software must be included ~ The unit must be DICOM conformant with VA standards and be able to transmit images to the VA VISTA IMAGING Server ~ The unit must be conformant with VA worklist standards and must have the ability to pull a worklist from the VA worklist servers ~ The unit must have all operating software, including image spectrum advanced tools, in order to operate and function fully ~ the system must be able to populate patient demographics on the device automatically when a clinical order has been placed in the VA consult system ~ Canon is being used now, this is an add on to what is already in place. Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. Delivery: Awardee shall coordinate with the Official Mail Center (OMC) prior to performance. FOB Destination- 30 Days After Receipt of Order (ARO). No remanufactured or gray market items. Buffalo VA Medical Center 3495 Bailey Ave Buffalo, NY 14215 EVALUATION: Evaluation will be determined by Lowest Price Technically Acceptable to the Government IAW FAR Subpart 13.106. Quotes must meet the technical standards set forth in the solicitation in order to be considered responsive. The Government reserves the right to make award without discussions.
