Description

THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY This is not a solicitation for proposals, proposal abstracts, bids, or quotations.   The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. SAO West NCO 17 Medical Sharing Branch II plans to solicit for a vendor who can provide services as part of its Community Based Health Care of Homeless Veterans (HCHV) program, and in accordance with the terms and conditions stated hereinto (see attached Performance Work Statement draft). SAO West NCO 17 Medical Sharing Branch II is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Place of Performance: Department of Veterans Affairs El Paso, Texas Type of Contract: If this request for information (RFI) leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope to this project. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization DUNS Number d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size under NAICS 624221 and/ or 624229 f. Socio-economic status (IF SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) g. Location in El Paso, Texas - only Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for the aforementioned NAICS codes. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than July 27, 2018 at 2PM, Mountain Time. Confidentiality: No proprietary, classified, confidential, or sensitive information should be included in your response.   The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
