- SF1449 which incorporates Addendum to FAR 52.212-1 and FAR 52.212-2. Financial Institution Reference Sheet Wage Determination Performance of Work Statement Past Performance Questionnaire Hurlburt Field has a requirement for the creation and implementation of a Learning Management System (LMS) which will result in the award of a firm fixed price contract with a base year and four option years. Please utilize the attached SF1449 which contains Addendums to FAR 52.212-1 Instructions to Offerors and FAR 52.212-2 Evaluation for Award. The following attachments are included with this SF1449 solicitation: 1. Performance Work Statement 2. Wage Determination 3. Past Performance Questionnaire 4. Financial Documentation Submissions are due to the government by 20 Aug 18 at 1200PM CST. Questions are due to the government, in writing, by 8 Aug 18. Please submit questions in writing to Mr. Cole Johnson, Contract Specialist, at Cole.Johnson.9@us.af.mil. PPQ's are due to the government by the solicitation closing date and time. Note: Hard copies will not be accepted for this acquisition. Please submit your proposal electronically IAW Addendum to 52.212-1 Instructions to Offerors.
