Page 2 of 2 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS, IN CONJUNCTION WITH FAR PART 13 SIMPLIFIED ACQUISITION PROCEDURES AND SUPLIMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. Solicitation 36C26218Q9744 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. In accordance with 38 USC 8127(d) Veterans First Contracting Program this solicitation is an SDVOSB set-aside. NAICS Code is 339113 and the Small Business Administration s (SBA) size standard is 1000 Employees. Only offers from contractors listed as SDVOSB on the VETBIZ website will be considered. The Department of Veterans Affairs, Network 22 Contracting Office is seeking sources for four hundred seventy five (475) disposable Bariatric Patient Repositioning Slings for the VA Long Beach Healthcare System. The disposable bariatric repositioning sling lies flat on top of the mattress, similar to a sheet, and is designed to make it easy to move or turn a bariatric patient or resident. The disposable bariatric patient repositioning sling is designed to take the place of the sheet and to be left in place on the bed. The sling should be positioned such that the head end is aligned with the head end of the mattress. The disposable bariatric patient repositioning sling is designed to allow a patient to be positioned in a supine manner while maximizing flexion in the hip area. It may also be used to log roll or turn a patient for frequent changes of position in bed and aides in reducing the risk of infection and contamination. The disposable bariatric patient repositioning sling must cover the entire top of the mattress. Bed sheets are not used and the patient will lie directly on top of the sling to decrease the layers between the patient and the mattress to prevent pressure sores. This requirement is being solicited as a Brand-Name or Equal solicitation. Offerors may propose to provide the following brand-name product or an equal product that meets each of the salient characteristics that are described below. Brand Name Product: Wy East Disposable Bariatric Patient Repositioning Slings. Model # 52082, 84 inches long, 42 inches wide, breathable material required to prevent bed sores. EQUAL PRODUCTS TO THE BRAND-NAME PRODUCT MUST MEET THE FOLLOWING SALIENT CHARACTERISTICS. OFFERORS PROPOSING TO PROVIDE AN EQUAL PRODUCT MUST PROVIDE A NARRATIVE EXPLANATION AND SUFFICIENT MANUFACTURER S PRODUCT LITERATURE TO DEMONSTRATE THAT THE OFFERED EQUAL PRODUCT MEETS THE SALIENT CHARACTERISTICS. OFFERORS WHO FAIL TO PROVIDE THIS INFORMATION WITH THEIR QUOTES WILL BE CONSIDERED NON-RESPONSIVE. QUOTES THAT MERELY STATE THAT THE OFFERED PRODUCT MEETS THE SALIENT CHARACTERISTICS WILL BE CONSIDERED NON-RESPONSIVE. Must be compatible with the following patient lifts: Guldmann s GH3+ ceiling-lift Hill-Rom s Likorall ceiling-lift Hill-Rom s Golvo 7007 floor-lift Hill-Rom s Golvo 8008 floor-lift Viking XL floor-based lifts Must be large enough to fully cover the top of the Hill-Rom Progressa mattress (between 82 86 inches long and 40 44 inches wide). Must be able to use on 2-point hanger bar and a 4-point hanger bar Must have 5 loops on each side of the sling 2 loops for head with 4 attachment points per loop for height adjustment 3 loops for body with 2 attachment points per loop for height adjustment Must be loop-style with color coded loops for correct height adjustment by staff Must be able to support a bariatric patient s weight up to 1000 pounds Must be disposable for single patient use Must be composed of breathable (cotton/polyester blend to absorb moisture from patients skin), air permeable (material that lets air pass) material to prevent bed sores; used on low air mattresses. Offers for an equal product must include: Color picture of the disposable Bariatric Repositioning Sling List of compatibility with the various lifts Map of pressure points that can accumulate moisture Place of Performance: VA Long Beach Healthcare System (VALBHS), 5901 E. 7th Street, Long Beach CA 90807 Period of Performance: Within 30 days of receiving award or sooner to ensure continued safe patient handling.
