- Statement of Work- Pureline Chlorine Dioxide Generators-FY18 The contractor shall provide the preventive and maintenance services for the following two (2) existing Pureline Chlorine Dioxide Generators, located at the Minneapolis VA Health Care System (VAHCS) facility: Pureline HP-10, serial #6010120, located in Building 69. Pureline HP-10, serial #6010119, located in Building 70- room BJ101. This agreement shall include the following services: Four preventive maintenance service visits per year, per generator, approximately 90 days in-between each service. One emergency visit per year, per generator. Up to two days on site for each visit (excluding the first visit for start-up). A preventive maintenance parts list shall be provided to the Minneapolis VAHCS. Travel and living expenses are included in the service agreement for each of the scheduled service visits. An initial start-up training for VA Facilities Engineering personnel will be required. An expected 3-4 Engineering technicians will be present. Due to time constraints and operational commitments, this training may be held two (2) different times, once at each Generator. Expected training to last between 2-3 hours per session. The first visit shall be planned to replace any parts that are defective. The funds for this visit shall be authorized by the Minneapolis VAHCS, and will be included as a separate line item. There shall be a line item for emergency repairs that fall outside the scope of preventive maintenance. The funds for this line item shall be used on an as needed basis and shall be authorized by the Minneapolis VAHCS facility before any work is performed using this line item. Each preventive service visit shall be scheduled in advance to coordinate availability for VAHCS personnel and the contractor. A notice of scheduled service shall be sent to the Minneapolis VAHCS at least 2 weeks prior to the visit. This notice of scheduled service shall also provide a list of the parts that will be needed for the Pureline Technician to perform their preventive maintenance tasks. The Contractor shall use testing equipment that is calibrated and adheres to the standards and criteria set forth by the National Institute of Standards and Technology (NIST), as recommended by the ISO-9001 requirements. As proof of current calibration, the Contractor shall provide a Traceable Calibration Certificate to the VAHCS, presented upon initial contract issuance and each time certificates are updated. Each Service Technician must be trained and qualified to repair the equipment they are dispatched to diagnose, repair and/or perform maintenance services upon. In order to provide proof that their qualifications meet industry and manufacturer s standards, the contractor shall provide a certificate of training for each Technician that may be dispatched to the VAHCS. The Contractor shall keep a material safety data sheet (MSDS) book on-site, at all times, for all hazardous materials being used or supplied for the Pureline generators. Upon request by the COR, the Contractor shall immediately provide the MSDS book. The Contractor shall follow OSHA and Minneapolis VA Medical Center lockout/tagout (LOTO) procedures. The Contractor is responsible for having an equivalent LOTO procedure to protect both the Contractor and VA personnel. In general, it is expected that the Contractor s procedures will conform to those of the VA policy. The VAMC lockout/tagout policy (Facilities Engineering Services Memorandum no. 28) will be provided upon contract award and whenever requested. The contractor shall provide their lockout/tagout policy, to the Minneapolis VAMC, upon contract award, and whenever requested. The Contractor will have and use at all times the manufacturers required Personal Protection Equipment (PPE) onsite. The Minneapolis VA Medical Center Facilities Engineering Department requests a base year with two (2) option years for this service contract. The dates for these contracts will be approximately: Base Year- August 10th 2018- August 9th, 2019 Option Year 1- August 10th 2019- August 9th, 2020 Option Year 2- August 10th 2020- August 9th, 2021 Any additional service time needed beyond the scope of preventive maintenance work shall be billed at posted service rates, which shall be submitted with the procurement package documents. Emergency service shall be provided within 48 hours of requested service. A service report shall be provided to the VAHCS for each service visit provided.
