Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY.    It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the  Government to award a contract.  Requests for a solicitation will not receive a response.  Responses to this Sources Sought must be in writing.  The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses interested and capable of providing the requested commodities. The NAICS Code is 339112; size standard is 1000 Employees. 100% VOSB set-aside. Responses to this notice shall include the following: (a) company name  b) address (c) socio-economic status, (d) point of contact (e) phone, fax, and email (f) DUNS number (g) Cage Code (h) Tax ID Number (i) and must  provide a capability statement that addresses the organizations qualifications and ability  to perform as a contractor and provide the commodities described below.       BIPAP VENTILATOR See Statement of Work below. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued.   Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.  Requests for information and resultant responses shall be submitted directly to the contracting officer, Wilmen Joa at Wilmen.Joa@va.gov. NO TELEPHONE CALLS WILL BE ACCEPTED. All submissions should be received no later than 11:00 am Eastern Standard Time on July 31, 2018. STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: BIBPAP/VANTILATOR, V60 TRANSPORT SONFIGURATION 2. Scope of Work: The contractor shall provide all line items listed within SOW. The requirement shall to be able to offer Ventilator V60 Transport Configuration package technology which will be used by Med/Surg Respiratory lab s unit to treat patients within James J. Peters VA Medical Center. 3. Background: The requirements will use in JJP VAMC throughout the inpatient/outpatient patients. The purchasing of the equipment will support patient safety, efficiency of patient care and positive healthcare outcomes due to the readiness/availability of the equipment. The purchasing of this equipment will also enhance staff efficiency and response to emergency timely and efficiently. This equipment is expected to impact cost savings by decreasing employee as well as impact patient satisfaction, confidence in the services provided by the team and the security of the equipment utilized for their care. Portable Ventilators Exclusives: 001. Ventilator, V60 Transport Configuration package 002. Service & Operational manuals 4. Performance Period: Vendor will be responsible for assembly of the unit. The vendor will also be responsible for shipping and handling. The contractor shall complete the work required under this SOW in _60__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer. 5. Type of Contract: The proposal shall be Firm-Fixed-Price B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the Contracting Officer has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. GENERAL REQUIREMENTS The vendor shall provide all line items listed above. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: Contractor shall provide all items listed within attachment A Price Cost Schedule. Vendor will be responsible for assembly of the unit. The vendor will also be responsible for shipping and handling. The contractor shall complete the work required under this SOW in _60__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer. Task One: The Contractor shall provide a detailed work plan and briefing for the Contracting Officers Representative (COR) which presents the contractor's plan for completing the task order. The contractor's plan shall be responsive with this SOW and describe, in further detail, the approach to be used for each aspect of the task order as defined in the technical proposal.
