- This is a REQUEST FOR INFORMATION (RFI) only. Market research is being conducted to determine the amount of qualified and capable businesses that can provide the items requested below. If you believe your firm is able to provide the item requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. Due date for responses is close of business (4:30PM local time/CT) on Wednesday, August 1, 2018. Email responses to Melanie Stockman at melanie.stockman@va.gov. All questions must be submitted in writing to the Contracting Officer in writing via email. Telephone inquiries will NOT be accepted/returned. When submitting a response to this RFI, offerors must include the following: Indicate socio-economic status Include picture of offered items and support literature, brochure etc Indicate any existing Government contract where item(s) may be purchased (i.e. FSS/GSA contract etc) Must be able to provide installation DISCLAIMER-This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. NAICS: 339112 Size Standard: 1,000 employees SPECIFICATIONS: Urodynamic Table, Quantity: 1 A table/chair is needed for the Urology clinic to perform urodynamic studies. The table must be on wheels and portable to accommodate space issues. Name brand or equivalent to Laborie Medical Technologies SON5021 Urodynamic Procedure Table. Salient Characteristics: 1. Quantity one: Urodyanmic Procedure Table Requires a minimum of 2 motors for Height and Back Angle Seat Height must have a range of 40 cm 100 cm (16 in 39 in) to accommodate positioning of disable patients. Chair Length 143 cm 193 cm (56 in 76 in); Chair Width: 77 cm (30 in) Chair must be portable with locking wheels that are at least 4 inches large. Must include seat cutout for voiding. 2. Quantity two each of the following accessories: Arm Rest (Right and Left) Leg Rest Leg rest holder Fold up Foot rest 3.
