Page 1 of 1 This is not a solicitation or request for quote (RFQ). This is not a request for proposal (RFP). This is sources sought notice to industry and the only information is posted on FBO web page. The attachments included on the FBO page do not include any additional information. If your company is interested please read this notice entirely and submit the required information, as requested herein. VA is seeking current qualified sources who have interest and capability of performing a one-time removal of 4500 gallons of aged/off specification E-85 gasoline for recycling. Along with the removal qualified vendor must clean 8000-gallon capacity tank along with fuel pumping system and all applicable filter replacements. This effort must be performed within accordance of all applicable federal (including VA regulations), state, and local regulations for the following location VA Salt Lake City Health Care System (VASLCHCS), at 500 Foothill Blvd, B.38 Salt Lake City, UT 84148. A qualified vendor is one who can provide all material equipment, supervision, training and supplies necessary to remove, recycle, and clean tank and pumping system. Additionally, vendor must provide and maintain all licenses and permits for operational personnel, trailers, vehicles and other equipment or resources required in compliance with all laws and regulations. Qualified vendor who can provide VA (VASLCHCS) with documentation that all recovered E-85 gasoline has been recycled in accordance to all local, state, and federal requirements. Qualified vendor is one who can provide Health and Safety Plan to the VASLCHCS Safety Office prior to commencing work. In the event of any spill caused by the vendor or the sub-contractor work the vendor will be responsible for the clean-up labor, equipment and personal protective equipment and the cost of clean up to satisfaction of the VASLCHCS Safety Office. Additional qualifications include recent experience of performing services of a comparable size, scope and complexity, as well as current and valid registration in SAM.gov. In your reply, please provide company DUNS number, company documentation of qualification to perform as described above, and state if you would be performing services as a primary contractor or subcontracting the services.
