Description
COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C26218Q9799 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as a Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 339112 and the Small Business Administration s (SBA) size standard 750 Employees. The Government is seeking a contract to acquire optimal vendor to provide stock supplies of CPAP masks, Mirage FX Nasal Mask with Grey Headgear, AirFit P10 Nasal Pillows Mask System, AirFit N10 Nasal Mask - Wide, AirFit F20 Full Face Mask with Headgear Large. This solicitation is for brand name only and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranties. Place of Performance: VA Greater Los Angeles Healthcare System (VAGLAHS), 11301 WILSHIRE BLVD Los Angeles CA 90073. Period of Performance: Within 10-20 days of placing order or sooner if available. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process.
