- This is a pre-solicitation notice for synopsis of "Preservation of the Force and Family" for Davis-Monthan AFB (DMAFB), Arizona. This is a commercial service; therefore, FAR Part 12, Acquisition of Commercial Items, shall apply to this acquisition. The contractor shall supply a physical therapist, athletic trainer, strength and conditioning specialist and clinical social worker for 2 locations - Portland, OR and Tucson, AZ. 8 employees altogether. Background: When a rescue squadron operator is degraded or de-conditioned, individualized and specialized care is required to rapidly restore his physical capabilities prior to returning him to full-mission-capable status. Hence, as professional athletesare supported by an integrated professional sports model that tailors performance programs to accomplish the mission at peak levels, operators require the same level of support to perform as tactical athletes and must have a comparable POTFF program. A successful POTFF program provides the following services: Physical Therapy/Injury Management, Athletic Trainer, Strength and Conditioning, and Clinical Social Worker. This service shall be provided in accordance with the Performance Work Statement (PWS). A Firm Fixed Price contract will result from this solicitation. This will be a Best Value acquisition using past performance, technical acceptability, and price for evaluation procedures. The North American Industry Classification System Code is 621340; and, the small business size standard is $7.5 million. The RFP Number is FA487718R0019 and shall be issued on or about 3 July 2018, posted electronically on the Federal Business Opportunity website ( http://www.FBO.gov ). No paper copies will be issued; no telephone request will be honored and no bidder's mailing list will be maintained. Potential offerors are responsible for monitoring this site for the release of the solicitation package and any amendments that may follow. Any proposal submitted is strictly voluntary. The government will not pay for any proposals submitted in response to the RFP. To be eligible for contract award, offerors must be registered in the Systems for Award Management (SAM). Register via the SAM website ( http://www.sam.gov ). Please Note: The subsequent award of this requirement shall not be held up by the government waiting for a firm to be properly registered. All responsible small business concerns may submit a proposal to the 355 th Contracting Squadron.
