This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99-1, 07/16/2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 611430, with a small business size standard of $11.0 Million. The Network Contracting Office (NCO) 9 is seeking to purchase high reliability organization (HRO) transformation services that consistently provides safe, efficient, effective veteran centered care and whose employees are high-performing team players who enjoy working in the organization for Tennessee Valley Healthcare System (TVHS). All questions regarding this RFQ must be in writing and may be sent by e mail to tara.hammrich@va.gov. Questions must be received no later than Aug 13, 2018 at 10:00 AM CST. All questions and concerns regarding this RFQ shall be directed to the Contracting Officer. Performance Work Statement High Reliability Healthcare Organization Transformation Services 1.0 DESCRIPTION OF SERVICES. The overall objective is to transform Tennessee Valley Healthcare System (TVHS) into a high reliability organization (HRO) that consistently provides safe, efficient, effective veteran centered care and whose employees are high-performing team players who enjoy working in the organization. The contractor shall provide High Reliability Healthcare Organization Transformation Services for Tennessee Valley Healthcare System and its associated facilities. The contractor shall demonstrate requisite experience in coaching and training comparably sized medical facilities. The contractor shall coach, train, and mentor TVHS leaders and staff on methods, techniques, processes, and supporting tools/templates that are statistically proven to produce sustained improvement (quality, safety, engagement, satisfaction, service, financial stability) at other large medical centers. Additionally, the contractor shall include statistically proven techniques, processes, and supporting tools/templates that produce sustained methods of accountability, achieve better operational efficiency / clinical outcomes and reduce process/outcome variability (clinical and administrative). The contractor shall provide documentation supporting the effectiveness of its techniques. The contractor shall implement techniques to improve patients' perception of care as evidenced by the VA Survey of Healthcare Experiences (SHEP) and VA Strategic Analytics for Improvement and Learning (SAIL) performance measures and improve employee satisfaction as evidenced by reduced turnover and improved all employee engagement survey (AES) results. The contractor shall implement techniques to improve operations, patient flow, wait times, safety, quality, finance and service in the Emergency Department. The contractor shall implement techniques to improve flow and throughput processes for more efficient, safer patient care and reduction in left without being seen rates. The contractor shall implement techniques to increase physician satisfaction, enhance doctor communication and reduce provider burnout. The contractor shall conduct an initial baseline evaluation and assessment of TVHS that will identify TVHS s strengths and opportunities for improvement. This assessment should include national leadership benchmarks. This assessment should be released from the Director s office and completed by all levels of TVHS s leadership structure. The contractor shall deliver the results of the assessment via an in-person presentation to the Director and facility leadership. The contractor should subsequently guide the leadership group through a discussion of recommended next steps. Following the baseline assessment, the contractor shall conduct quarterly follow-up assessments in comparison to the baseline evaluation and assessment to validate progress toward the organization s objectives. Baseline and Quarterly follow up assessments will assess: Alignment of leaders actions to improve SAIL, AES and SHEP scores with trending. Current level of accountability among leaders and trends. Progress of training and improvements in customer service and leadership skills. The contractor shall conduct a standardized monthly support meeting to provide TVHS leadership coaching, teaching and mentoring either in person, virtually or a combination of both. The contractor shall develop and deliver quarterly training seminars to TVHS leaders and staff designed to provide hands-on learning opportunities to supplement the in person and virtual coaching. The contractor shall work with a team of TVHS leaders to customize curricula for each quarterly training. The curricula will include, but not limited to, leader accountability plans, focused leader rounding, leading best practices to improve patient communication, Leadership training will be provided to up to 1000 leaders at TVHS in class sizes of no more than 75 per live session at each campus and virtual sessions as possible. Actual number of participants to be mutually agreed upon by TVHS and the contractor after the initial assessment. The contractor shall provide and implement an established robust, automated, web based rounding tool for use by all leaders. This tool must provide: templates for hourly and other Veteran-centric rounding techniques templates for rounding on employees capacity for customization, allowing instantaneous, immediate creation of custom templates and stoplight reports The contractor shall provide and implement a web-based leader accountability tool capable of delivering real time reports. This tool must allow TVHS leaders to enter, access, and share goals and progress data transparently and efficiently. The contractor shall also establish an accountability implementation team comprised of TVHS s leaders. This tool must contain functions that: Establish weighted, measurable goals for leaders Enable quarterly action planning Facilitate monthly support meeting discussions Validate the efficacy of action planning The contractor shall provide onsite coaching visits. The contractor will have coaches and mentors available with specific operational expertise and demonstrable prior experience in leadership best practices, communication templates to improve patient/employee trust, shift pass down protocols to ensure smooth transition between shifts, cultural change, change management, healthcare finance, emergency department management, employee engagement, physician engagement, Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) improvement, leader development, executive coaching, executive rounding and quality improvement. For the duration of the period of performance, the contractor must provide TVHS with an online catalog of best practices, tools, workbooks, books, and other resources that have been harvested, tested, and refined from other healthcare organizations. Throughout the period of performance, the contractor shall provide access to all TVHS leaders the leadership development and automated accountability systems, tools, publications, video and online resources required to achieve and sustain the desired outcomes. The contractor must be able to provide TVHS with leading best practices from industry inpatient, emergency department, physician and employee engagement arenas. Performance Measures: Contractor s baseline assessment shall be used to assess progress achieved over time. Quarterly, contractor shall provide metrics on number of staff trained and a summary of an end of training questionnaires provided to staff who were trained that quarter. This questionnaire should discuss perceived usefulness of training, quality of instruction, applicability to the workplace and suggested changes to the training for next quarter. Annually, contractor shall reassess their techniques via survey of all TVHS staff who have participated and present findings to executive leadership. LOCATION OF WORK. The contractor shall perform assessments, coaching, and training at TVHS campuses, its associated facilities or other coordinated venue. TVHS is classified as a National Disaster Medical System hospital for the Nashville Federal Coordinating Center, and is a Level 1A, integrated tertiary healthcare system comprised of two hospitals, the Alvin C. York Campus in Murfreesboro, TN, and the Nashville Campus in Nashville, TN. TVHS has 20 community based outpatient clinics / sites of care located in Tennessee and Kentucky. TVHS supports a health service catchment area encompassing parts of 3 states, Tennessee, Kentucky and Georgia. TVHS provides ambulatory care, primary care, and secondary care in acute medicine and surgery, specialized tertiary care, transplant services, spinal cord injury outpatient care, and a full range of extended care and mental health services. TVHS s Nashville Campus is the only VA facility supporting all solid organ transplant programs, including total in-house kidney and bone marrow transplants and is a national referral site for bone marrow and solid organ transplants. The York Campus is a network referral center for mental health services, long term psychiatric care, geriatrics, and extended care. The Department of Defense (DoD) and TVHS operate a community-based clinic at Arnold Air Force Base / Arnold Engineering Development Center (AEDC) in Tullahoma, Tennessee. The Nashville Campus is a primary receiving facility in support of the VA/Department of Defense Contingency Hospital System during times of war or a national emergency involving military personnel. TVHS currently partners with Fort Campbell Army Post as part of the Individual Disability Evaluation System (IDES). IDES is part of an ongoing effort to accelerate the elimination of the disability compensation claims backlog for claims of Wounded Warriors separating from the military for medical reasons. 1.2. HOURS OF OPERATION. The contractor shall perform assessments, coaching, and training during normal duty hours which normally range from 7:00 a.m. to 5:00 p.m., Monday through Friday, excluding Federal holidays unless otherwise required in the contract or approved by the Contracting Officer. 1.3. Oct 01, 2022 to Sep 30, 2023 12 Months TOTAL PRICE For the purposes of the award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time, and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract.   The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised.   The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise.   This evaluation will not obligate the Government to exercise any option under FAR 52.217-8. Technical Requirements that the Contractor Shall Provide: Technical: The offers proposal should address each area of the Performance Work Statement (PWS) in sufficient detail to demonstrate a firm understanding of the requirements and goals set forth in the PWS. The technical solution must provide a complete plan to execute and accomplish the requirement defined in the PWS. Following receipt of quotes, the Government will perform an evaluation using a comparative analysis of the services quoted. The Government will compare quotes to one another to select the service that best benefits the Government by fulfilling the requirement. The evaluation of Government requirements outlined in this request for quote will determine suitability. Once the Government determines there is/are a contractor(s) that can provide a service that meet(s) the requirements of this request for quote, the Government reserves the right to communicate with only those contractors quoting the best-suited service to address any remaining issues. A copy of the designation shall be furnished to the contractor 852.273-74, AWARD WITHOUT EXCHANGES (JAN 2003) The Government intends to evaluate proposals and award a contract without exchanges with offerors. Therefore, each initial offer should contain the offeror s best terms from a cost or price and technical standpoint. However, the Government reserves the right to conduct exchanges if later determined by the contracting officer to be necessary This is an open-market combined synopsis/solicitation for High Reliability Healthcare Organization Transformation Services as defined herein.    The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s). Submission shall be received not later than Aug 17, 2018 at 10:00 AM central via email to tara.hammrich@va.gov.
