- THIS IS A PRE-SOLICITATION NOTICE. The Request for Proposal (RFP) Number will be 36C79118R0015. The Department of Veterans Affairs (VA), Commodities & Services Acquisition Service (CSAS), 555 Corporate Circle, Golden, CO 80401, intends to solicit for offers from qualified and experienced vendors with analyzing capabilities for rehabilitation outcome standards using Standardized Functional Outcome Metric (SFOM) through full and open competition. The intent of the RFP will be to award a single indefinite delivery / indefinite quantity contracts for Standardizes Functional Outcome Metric. The VA need to document the severity of patient disability and to establish rehabilitative outcome standards based on each severity level. VA seeks to acquire access to a SFOM, and through the use of the SFROM have access to the results of reporting. Data collected within VA using the Standardized Functional Outcome Metric and submitted to the contractor through secure data transfer procedures will be applied to key patient functional attributes (indicators) and the results fed back to both facility-level rehabilitation managers and national program-level managers on a quarterly basis for analysis. Government s requirements are per the following: Provide the VA with recognized rehabilitation outcome standard using SFOM which can be utilized to demonstrate inpatient rehabilitation outcomes; Provide training and credentialing rehabilitation programs for usage of a SEOM which can be used to demonstrate inpatient rehabilitation; Have experience in collecting, analyzing and performance reporting utilizing a SFOM for inpatient rehabilitation programs; Compliance with all regulatory, privacy and safety regulations required for providing healthcare services; and Have the ability to send and receive secure file transfers from any federal agency. The Government intends to execute a single award contracts with a base and four (4) options year. This acquisition will be conducted as a full and open competition for any and all qualified, experienced, and knowledgeable business entities, whose offers represent the Lowest Price Technically Acceptable. The North American Industry Classification System is 561110, Office Administration Services. The Small Business Size Standard is $7.5 million. Solicitation documents will be available electronically in the near future through the Federal Business Opportunities System (FedBizOpps) website at http://www.fedbizopps.gov. The anticipated closing date is August 31, 2018. The Contracting Officer for this acquisition is David Little. Upon review of the solicitation documents, prospective offerors are encouraged to provide questions and/or comments in writing. All contractors must be registered System for Award Management (SAM) website as an active contractor in order to be eligible to enter into a contract and to receive payments. The SAM website is found at https://www.sam.gov/portal/public/SAM. Please be sure your company is registered at this website. Prospective offerors should complete electronic annual representations and certifications, which is also part of the SAM.
