SOURCES SOUGHT NOTICE Community-Based Outpatient Clinic (CBOC) Yreka, California This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources such as Service-disabled veteran-owned small firms, Veteran-owned small firms, Hubzone firms, general small business firms and their size classification relative to NAICS 621498 (size standard $20.5 million). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. NOTICE: The U.S. Department of Veterans Affairs, VA Northern California Health Care System is seeking sources for a Primary Care CBOC in Yreka, CA. The VA anticipates soliciting for an Indefinite Delivery, Indefinite Quantity, Capitated contract. The following services to be provided in a private hospital, office or clinic environment to veterans, primarily residing in Siskiyou County and the surrounding area of Northern California. DESCRIPTION: Primary Care CBOC is defined as one that: offers medical (physically on site) and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide primary care services. Access to specialty care is not provided on site, but may be available through referral or telehealth. A Primary care CBOC often provides home-based primary care (HBPC) and home telehealth to the population it serves to meet the primary care and mental health needs of Veterans who have difficulty accessing clinic-based care. These clinics have access to a higher level of care within a VHA network of care. Primary care in VA includes both medical and mental health care services, as they are inseparable in providing personalized, proactive, patient-centered health care. VA will provide the mental health services. SUBMISSION: Potential offerors will be required to demonstrate their total solution for establishment and operation of a CBOC to include providing and building out space for a medical facility, staffing, and overseeing day to day operations of the CBOC. The resultant medical care provided will be expected to be seamless in that a beneficiary cannot differentiate between the quality of care and services provided at a VA operated CBOC and the Contractor operated CBOC. All interested parties should respond electronically (via email attachment) no later than 4:00 p.m. Pacific Time on August 20, 2018, to the VA s representative, Tracy Worrell at tracy.worrell@va.gov, subject: Yreka CBOC Sources Sought Response. Interested firms having the skills and capabilities necessary to perform the stated requirements are invited to provide information to contribute to the market survey/sources sought notice. Responses to this Notice should be limited to five pages and must provide the following information: (1) General Information of the company name, address, phone number, fax number, e-mail address(s), and company web site. (2) How long has your company been in business? (3) What services does your company primarily provide and for how long. (4) Does your company have experience with the Department of Veterans Affairs capitated CBOC program? (5) What relevant Government and commercial experience does your company have related to establishment of CBOCs? (6) Describe how you intend to provide the Capitated CBOC services for our Veterans. (7) Describe your company ownership composition listing the names of individuals that control and have authority to enter into a contract with the Government and their percentage of ownership of your company. (8) Will your company be establishing a joint venture with another company? If so, list the names and provide the company s general information, their responsibilities, and authority of managing of the contract. (9) Does your company meet the standards of Joint Commission (JC) (10) Is your business small relative to NAICS 621498, size standard $20.5 million? (11) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (12) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (13) Is your firm a certified "Hubzone" firm? (14) Is your firm a woman-owned or operated business? (15) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran owned small business? (16) Provide your Dun and Bradstreet Number.
