- 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation number IHS1360521 is being issued as a Request for Quotation (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-57. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This solicitation is a total small business set-aside. The North American Industry Classification System (NAICS) code is: 339114 - Dental Equipment and Supplies Manufacturing 5. This requirement consists of two (2) line items: 0001 - Single File Reciprocating System 0002 - Miscellaneous Files and supplies associated with the Single file Reciprocating System 6. The Statement of Work is attached to this notification. The requirement is for the purchase of 4 Single File Reciprocating Systems with associated attachments (listed in SOW). SOW does seek for Name Brand or Equivalent. The Government reserves the right to determine if a product is in fact equivalent. 7. The location of contract performance Hopi Health Care Center, Highway 264, Mile Marker 388, P.O. Box 4000, Polacca, AZ 86042, and the point of delivery is FOB Destination to the same address. The Period of Performance is 45 Calendar Days after issuance of Notice to Proceed. 8. INSTRUCTIONS TO OFFERORS: 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation. Offerors must submit a technical description and proposed shipment of items with sufficient detail to allow the contracting officer and technical evaluators to conduct the evaluations and make determinations based on the established evaluation criteria. 9. PROVISIONS/CLAUSES: 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered: I. Technical a. Items shall accomplish endodontics in providing patient dental care II. Past Performance a. Provide three references (include name, telephone, address and email address) III. Price The evaluation factors are listed in descending order of importance.
