- Solicitation W81K0218Q0168 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Fort Shafter, HI 96858, intends to solicit and award a purchase order for Cheetah Starling TM SV Non-invasive Cardiac Output and Hemodynamic Monitoring System and Sensors (Brand name or equal) for Tripler Army Medical Center (TAMC), Hawaii. This is a Request for Quotation (RFQ). This solicitation number is W81K0218Q0168. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. The Standard Industrial Classification Code is 6515, size standard is 1,250 employees and the NAICS code is 334510. This procurement is a total small business set-aside, brand name or equal. Provisions at 52.212-1, Instructions to Offerors - Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, and its addendum applies to this acquisition. The deviation to clause 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executives Orders - Commercial Items, applies to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Purchase Order. Contract Requirements - The successful offeror shall be required to provide the brand name or equal for Cheetah Starling TM SV Non-invasive Cardiac Output and Hemodynamic Monitoring System and Sensors for TAMC, Hawaii.
