Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format of FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested (RFQ) and a written solicitation will not be issued. The solicitation number NIH-CC-OPC-18-012440 and is being issued a Request for Quotation (RFQ). The North American Industry Classification System (NAICS) for this procurement 811219. Background Information The National Institutes of Health (NIH) is the nation's foremost federally funded biomedical research institution. It is comprised of twenty-seven Institutes, Centers and Divisions. The Warren G. Magnuson Clinical Center is the on-site 267-bed hospital for the NIH campus comprised of clinical, laboratory and administrative staff who support the Institutes as they conduct clinical research working with inpatients and outpatients participating in research studies at the NIH. The Department of Peri-Operative Medicine (DPM) provides surgical services support for patients enrolled in the experimental protocols of several Institutes. The National Institutes of Health (NIH), Clinical Center (CC), Department of Perioperative Medicine Section (DPM) has a need to obtain a vendor to repair surgical instruments and positioning equipment required for on-going experiments. Repairs may consist of the following: Aloka DaVinci Ultra Sound Probe Aloka Laparoscopic Ultra Sound Probe Aloka Neuro Aloka Ultra Sound Probe Aloka Finger Probe Sternal Saw Stryker System 5 Power Various Laparoscopic Instrument Repairs Ridgid Scopes Flexible Bronchoscope Flexible Video Bronchoscope Flexible Ureteroscope Flexible Cystoscope DESCRIPTION Flexible Reno Veno Urology Scope Ebus Scope Dipping of Various Surgical Instruments Various Surgical Instrument Repairs Repairing of Surgical Instrument Sterilization Pans Repairing of Light sources Repairing of head lights Repairing of Surgical Instrumentation Tables Replacement of Rubber pieces to Surgical ENT and Dental Instruments Repairs to Airplane Arm for Patient Positioning Repairs to Either Screen Repairs to Laser Instruments Modification to Neuro Suction So that it is Crimped Lengthen of Surgical Instruments as needed Repair to Buddy Halo Retractor Repairs to Thompson Retractor Repairs to Harmonic Scalpel Cords Repairs to Diamond Knife for Neuro Repairs to Armboards Company will pick up and drop off on a daily basispe Period of Performance Base and Three Option Renewels August 25, 2018 through August 24, 2019 August 25, 2019 through August 24, 2020 August 25, 2020 through August 24,2021 Capability Statement is to include: Information regarding respondents; (a) staff expertise, including their availability, experience and informal and other training; (b) current in-house capability to perform the work; (c) prior completed projects of similar nature; ; and (e) examples of prior completed Government contracts; references, and other related information: • Respondents, DUNS number, organization name, address, point of contact, and size and type of business (e.g.8(a), HUBZone, etc) pursuant to the applicable NAICS code; Technical Evaluation Elements Respondents must respond to this notice 1. Technical Approach (25) The Government will evaluate the offeror's technical approach to satisfy the following solicitation requirements. Preventative Maintenance duties. Telephone Support duties. Pick up and Drop off Protection. 2. Experience(25) The offeror must demonstrate a minimum of two (2) years of experience providing repair of surgical instrument services similar to or equivalent to this procurement. 3. Past Performance (25) The offeror must provide three references from previous customers where the offeror provided maintenance support with similar specifications and complexity. 4. PRICE(25) The price quoted will be evaluated for price reasonableness. A best value analysis will be performed taking into consideration the results of the technical evaluation, price evaluation, and the ability to complete the work within the Government's required schedule. Contractors must respond by August 24, 2018 11:00am Eastern Time.
