Description
This is a sources sought for the PP VAMC to determine the availability of potential sources (and their size classification relative to NAICS 541380 having the skills and capabilities necessary to provide Potable Water Testing Services in accordance with the attached SOW. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. No award will be made from this sources sought. Requested information should be submitted by email to vanessa.jacobs@va.gov by 12:00pm EST August 27, 2018. Provide only the requested information below. REQUESTED INFORMATION STATEMENT OF CABABILITY: Submit a brief [ Five (5) pages or less] capability statement explaining your capability to perform services for the DC VAMC. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility.. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (c) Include the DUNS number of your firm. (h) Provide your firms FSS or GSA Contract number if applicable. Statement of Work INTENT Provide all labor, equipment, and materials necessary to collect and test potable water samples throughout the Department of Veterans Affairs Medical Center (VAMC) Perry Point campus as required by Maryland Department of Environment (MDE) and VA Maryland Health Care System (VAMHCS) with respect to rural drinking water production systems. All samples must be collected and tested according to MDE specifications and requirements. The sample period is from October 1,2018-September 30,2019. The C&A requirements are not required. LOCATIONS The samples shall be collected at various points of use throughout the VAMC Perry Point campus located at the following address: VA Medical Center Perry Point, Maryland 21902 SAMPLING AND TESTING REQUIREMENTS A. The number of samples collected and the point of use locations will vary on a monthly basis according to MDE and VAMHCS requirements and direction. B. On an annual basis, the estimated minimum number and types of tests required is as follows: Description Quantity/Frequency Bacteria/Total Coliforms (E-Coli) 24 per year/2 per month NO3 for drinking water SM 4500D 1 per year (3rd qtr) Alkalinity 12 per year TOC (Total Organic Carbon) 12 per year HAA5 (Haloaceitic Acids Testing) 8 per year THM (Trihalomethanes) 8 per year Metals (Phase 2 & Phase 5) 1 per year Lead/Copper 1-10 per year (or as per regulation) Arsenic EPA 6010/200.8 1 per year Fluoride EPA 340.2/SM4500F 1 per year SOC(Method 525) 2 Qtrs./yr. June/Sept. C. Provide a written report for each sample taken and test conducted, summarizing the results, deficiencies, and recommended corrective actions. The written reports shall be submitted within 10 calendar days of collection of potable water samples. All reports, including any reports to Maryland Department of the Environment (MDE) in duplicate, shall be mailed directly to the COTR. D. Water sample collection must be coordinated in advance through the COTR who is: James K. Willen Water Treatment Plant Supervisor Facilities and Engineering Service VA Medical Center Perry Point, Maryland 21902 Telephone: (410) 642-2411 extension 5918 IV. CONTRACTOR PERSONNEL A. Certification of Testing Laboratory. The contractor shall operate or utilize a testing laboratory for the samples collected under this contract that is certified by and meets the minimum requirements of State of Maryland MDE and health department. Certified laboratories are required to follow EPA-approved protocols regarding the handling and analysis of compliance samples. B. Personnel Experience. All work shall be performed by competent technicians, experienced and qualified in collecting and testing potable water. The Contractor shall submit to and for the approval of the Contracting Officer, certification and a list of credentials of the approved technicians to be used for this work. Personnel shall have a minimum of three (3) years recent experience related to this work. The contractor must provide a list of three (3) reference contacts that have been provided similar service. The contractor must provide authenticated training certificates or credentials (or acceptable substitute as approved by the COTR) for any personnel who are collecting and testing any VA water samples. The VA specifically reserves the right to reject any of the contractor s personnel and refuse them permission to work on VA premises. C. Conduct of Personnel. The Contractor shall ensure that his personnel present a professional appearance at all times and that their conduct shall not reflect discredit upon this facility or the Department of Veterans Affairs. The Contractor shall remove from the job site any employee for reasons of misconduct or security. The removal from the job site of such a person shall not relieve the Contractor of the requirement to provide sufficient personnel to perform adequate and timely service. All employees shall be legal residents of the United States and be able to understand, speak, read and write the English language. D. Identification of Employees. : The Contractor shall within 10 calendar days before beginning performance of work provide the Contracting Officer a list of all employees who will perform under this contract. The list shall include the full name, and work assignment of each employee. The Contractor shall notify the Contracting Officer in writing of any addition, deletion or change in work assignment within 10 days of such change. Each of the Contractor's employees shall conspicuously display on their person a badge provided by the Contractor that shall include the full name of the employee, the name under which the Contractor is doing business and a badge serial number. V. HOURS OF OPERATION A. Normal Duty Hours. The Contractor shall have employees available from 7:00 AM to 4:30 PM, Monday through Friday, except legal public holidays to perform routine work. B. Federal Holidays. Federal Holidays observed by the VA are: New Years Day; Martin Luther King Day; President s Day; Memorial Day; Independence Day; Labor Day; Columbus Day; Veterans Day; Thanksgiving Day; and Christmas. CONTRACTOR QUALITY CONTROL The Contractor shall establish and maintain a Quality Control Program that ensures that all requirements of the contract are provided as specified. VII. INSPECTION AND ACCEPTANCE A. Final inspection and acceptance by the Government shall be made at the site of each system. However, final inspection and acceptance do not preclude the Government's right to inspect completed work prior to shipment from the Contractor's shop nor does it waive any rights of the Government. B. Unsatisfactory service will be reported in writing by the CO to the Contractor. C. Telephone Answering Service: The Contractor shall maintain, at his own expense, a dedicated (tie-line) toll-free telephone answering service for the exclusive use in the performance of this contract. This answering service shall be monitored during normal work hours as identified in the specifications. VIII. REMOVAL AND/OR DISPOSAL Removing Government-Owned Material: All property removed remains the property of the U.S. Government and shall be returned by the Contractor to a place of storage as designated by the CO or COTR. FIRE PREVENTION AND PROTECTION The Contractor shall fully acquaint himself and his personnel with the location of fire equipment before work, take all prudent measures to minimize fire hazards that result from work, complies with all ordering agency fire and safety regulations. ENERGY CONSERVATION The Contractor is required to comply with all energy conservation regulations and instructions that are applicable to Government-owned facilities including but not limited to thermostat settings. CLEANING UP The Contractor shall clean up after work, leaving each area where work is performed, neat and clean. All containers, trash and debris for which he is responsible shall be removed. HAZARDOUS CONDITIONS During the course of performing work under this contract, should the Contractor find serious or hazardous conditions existing with or in any of the equipment the Contractor shall secure the equipment and immediately notify the COTR. CLEANLINESS The area immediately around the equipment provided for the Contractor s use shall be kept in a clean and orderly condition. Trash and waste material shall not be permitted to accumulate, but shall be removed by the Contractor s employees. XIV. INSURANCE A. Worker compensation and employer s liability. Contractor and any subcontractor covered under this contract are required to comply with applicable Federal and State Worker s Compensation and occupational disease statutes. B. General Liability. Contractor and any subcontractor covered under this contract are required to have Bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence. C. Property Damage Liability. Contractor and any subcontractor covered under this contract are to have Property Damage Liability insurance coverage of at least $500,000. D. Contractor shall provide proof of liability insurance within five (5) days after contract award. SMOKING POLICY Smoking is prohibited inside all buildings except in designated areas. BILLING/PAYMENTS Each invoice shall contain the documentation required by this contract. The invoices shall be itemized to contain the number of samples collected and the number and type of each test conducted. Contractor shall enter all invoices into the VA s Electronic Invoice Presentment and Payment System (IPPS).( See website at http://www.fsc.va.gov/einvoice.asp Payment. Payment will be made 30 days in arrears upon receipt of properly prepared invoice. Invoice must contain the contract number and the appropriation number. XVII. VENDOR REQUIREMENTS 1. The contractor must operate or utilize a testing laboratory that meets State of Maryland health department and Maryland Department of Environment (MDE) minimum requirements for testing of potable water quality. 2. The contractor must have at least 5 years experience satisfactorily performing such collection and testing of potable water.
