- Specifications of HDTV Scope of Work Worksheet for Price Schedule This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for offers for a contractor to provide and install digital headend for the Information Resource Management (IRM). CCHCF has identified a specific brand name, but will accept brand name or equal descriptions per FAR 6.302-1(c)(2). Services shall be performed in strict accordance with the attached Scope of Work (SOW). CCHCF intends to award a firm fixed price contract as a result of this solicitation. Delivery Terms: ARO 30 days The Chinle Comprehensive Health Care Facility is a hospital providing medical care for approximately 35,000 members of the Navajo Nation. CCHCF provides both outpatient and inpatient services, with 60 bed inpatient hospital beds. Services offered are: Adult inpatient and Pediatric Inpatient Care, Outpatient Primary Care, Adult Intensive Care, Emergency Medicine, General Surgery, Podiatry, OBGYN, Labor & Delivery, Women's Health, Midwifery, Mental Health, Pharmacy, Optometry, Dental, PT, OT, Speech Pathology, Audiology, Lab, Public Health and School Health. EVALUATION CRITERIA The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance, Experience/Qualifications, and Logistics. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. SUBMITTAL CONTENT To be considered for award, your offer shall include the following documents: 1) Complete and sign the attached Pricing Schedule. The Government will consider competitive rates upon negotiation. 2) Provide your Quotation. 3) Provide evidence in experience providing services to Federal Government or other large commercial/government entities within the last three (3) years of operation. 4) Each Bid Response must include the following specification details: - Manufacturer and Model of item quoted  -Delivery Lead Time after receipt of order  -Warranty Information  -Line Item Unit Prices  -Total Price Email offer directly to the Contracting Officer for this action is: Ken Parrish, Procurement Technician, 928-674-7634, ken.parrish@ihs.gov.
