SEEKING ANY VETERAN OWNED SMALL BUSINESS (VOSB) WHO COULD PROVIDE REPLACEMENT PARTS FOR A SINTEK STOCKFLOW CARROUSEL PHARMACY STORAGE SYSTEM FOR THE OUTPATIENT PHARMACY AT THE EDWARD HINES JR. VA HOSPITAL, HINES, IL. PLEASE SEE THE STATEMENT OF WORK BELOW FOR FURTHER DETAILS. Project Name and Contract Title: Outpatient Pharmacy Bldg. 200 Replacement Parts 1) Objective. The goal of this project is to replace and add parts to the existing pharmacy storage system in Outpatient Pharmacy, Bldg. 200. The goal is to match existing parts to ensure that the replacement parts fit into the existing furnishing system. Provide replacement parts, brackets, trays, shelves, drawers and partitions and match existing product in space. Replace counter tops and additional ergonomic stools. 2) Scope of Work. The scope of work for this project includes manufacture, delivery and installation of new storage replacement parts. It also includes all management and materials. The existing system on site is a metal frame with incremental connection points for direct connect of these requested parts. The system is not universal & requires proprietary parts for assembly. It also includes all management and materials. The storage replacement parts must consist of the following design criteria selections of the following recommended manufactures: Stock Flow, Sintek: Itab for pharmacy storage replacement parts. All products must meet the salient and technical specifications defined in this section. Equal or Approved Equal. Product List: Qty: 6 Shelf: Shelf with Built in brackets, including plastic front. Can be placed level or sloping plastic front included. Product No: 340901 Height: 77mm or 3 Width: 792 mm or 31.18 Depth: 300 mm or 11.81 Front: 40mm or 1.5 Qty: 6 Drawer: Drawer with brackets, fully extendable with Metal front. Product No: 331001 Height: 200mm or 7.8 Width: 792mm or 31.18 Depth: 385mm or 15.15 front: Sheet Metal Qty: 70 Partitions: 30 mm partition of transparent environmental friendly plastic Product No: 442537 Height: 30 mm or 1.18 For shelf with depth: 200mm or 7.87 QTY: 4 (see count and sizes below) Laminate thermo fused counter tops: Rectangular counter tops (worksurfaces) have a thermo fused laminate top with a vinyl edge. Counter tops are 1/ 14 thick install backslash onsite. All support brackets must be provided as well as structural metal cross supports under the counter tops for lengths longer the 48. See photos of existing provided below. 2- 96 W x 24 D with a detached 5 H backsplash to be attached onsite 1- 54 W x 24 D with a detached 5 H backsplash to be attached onsite 1- 66 W x 24 D with a detached 5 H backsplash to be attached onsite Bracket under counter top. Installation place /Policy/Period of Performance/Procedures. Furniture is to be delivered to the Warehouse BLDG. 220 at Edward Hines VA Hospital, 5000 S. 5th Ave., Hines, IL 60141 and installed at Edward Hines VA Hospital, 5000 S. 5th Ave., Hines, IL 60141. Work is to be performed during normal business hours: Monday through Friday, 8AM-3PM.
