- Ultrasound Kiosk (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.603, as supplemented with additional information included in this notice. This announcement includes a written solicitation (SF 1449) for quotes. (ii) This solicitation document, RFQ - H92239-18-Q-0026, is issued and attached as a Request for Quote (RFQ), along with Attachment 1, End Product Listing. (iii) This solicitation document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-100, effective 20 Aug 2018. (iv) This requirement will be procured as Small Business set-aside. All eligible firms may submit a quote. The North American Classification System (NAICS) code is 339112 "Surgical and Medical Instrument Manufacturing", size standard is 1000 employees. This NAICS code was selected as it best describes the requirement. The selected PSC code is 6515. The policies and procedures under the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, in conjunction with FAR Subpart 13.5, Simplified Procedures for Certain Commercial Items, will be utilized for this procurement. (v) Prices shall be listed for each Contract Line Item Number (CLIN) of this RFQ (see attached Solicitation H92239-18-Q-0026) (vi) This requirement is to provide ultrasound equipment for HQ US SFG located at Fort Bragg, NC. The specific equipment is used to rehabilitate soldiers of the U.S. Army Special Operations Command. (vii) Items shall be delivered to Fort Bragg, North Carolina. (viii) For Provision and Clauses that applies to this acquisition see the solicitation, H92239-18-Q-0026.
