Description
SOW Amendment 1 - Combined Synopsis & Solicitation **Amendment to revise Proposal Submission Date to 11:00 am EST on September 14, 2018.** Title: Services to assist CMS with tasks related to the ongoing development and projected redesign of the CLIA database (application, surveys, billing, complaints, proficiency testing, enforcement and the reporting system) that is part of the QIES data system. Description: This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemental with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION: A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid (CMS) intends to award on a competitive basis, under the Simplified Acquisition Procedures in accordance with FAR Part 13 (Dollar Threshold of $250,000), a firm fixed price award for a project entitled, "Development & Redesign of CLIA Database." A detailed Statement of Work (SOW) and Deliverables are attached. Responses shall be no more than 10 pages and shall include the following: 1)Technical Understanding and Approach The offeror's quotation is complete and demonstrates an understanding of the SOW including an awareness of the contract objectives and the tasks required that produce the desired product. The proposed approach must be (i) sound, (ii) sufficiently documented; (iii) incorporate appropriate use of actuarial resources, and other professionals, if applicable; and (iv) and ensure the achievement of timely and acceptable performance. 2)Personnel The offeror's quotation shall demonstrate staff with the necessary skills to meet all requirements. Key personnel must be available to support all work under the effort and resumes of staff and consultants shall demonstrate not only academic / professional qualifications but also actual experience in similar tasks. If subcontractors are proposed, information is provided to support the qualifications of the subcontractors. 3)Management The offeror shall demonstrate the ability to manage all aspects of the proposed effort including technical performance, time and delivery constraints, cost, status reporting, and subcontractor(s). Ability to identify, address, resolve, and communicate problems (including conflicts of interest), and to maintain a positive working relationship with CMS staff are important. 4)Past Performance - The offeror shall provide past performance information on successful performance in a variety of projects of a similar magnitude and type (size and technical area) as defined in this SOW. 5)Price - Fully Loaded Labor Rate It is anticipated the number of labor hours will be 900 hours. The award will be made on the basis of lowest priced most technically acceptable (LPTA) meeting or exceeding the acceptability standards for non-cost factors. All responsible sources must be registered in the System for Award Management (www.sam.gov). The anticipated period of performance is one twelve-month base period and one twelve-month option period. The North American Industry Classification Code (NAICS) is 541519. QUESTIONS WILL NOT BE ANSWERED BY TELEPHONE. All responses/quotes submitted shall remian valid for a period of 180 days. Interested parties myst be able to affirm no conflicts of interest with parties involved in Medicared claims processing in light of the work identified in the Statement of Work. In the event that the vendor/Government identifies a conflict, the vendor will be responsible for providing a mitigation strategy that satisfies the Governemnt. In the event the conflict cannot be resolved, the order cannot be awarded or an existing order may be terminated.
