- PWS FAR Clauses Wage Determination Combined Solicitation FBO SOLICITATION 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2. The solicitation number is W912JB-18-Q-0156 and is being issued as a request for quotation (RFQ). 3. This solicitation document and incorporated provisions and clauses in effect through Federal Acquisition Circular 2005-95. CLIN Description Quantity U/M 0001 Medical Surveillance 1 Job 0002 Contractor Manpower Reporting 1 Job 4. The Michigan Army National Guard has a requirement for medical surveillance from a medical facility within 50 miles of 3423 N Martin Luther Kind BLVD, Lansing, MI. Please see the attached Performance Work Statement for exact requirements (PERFORMANCE WORK STATEMENT, Pages 16). This requirement is firm fixed price and the period of performance is 12 months. 5. Offers are due at 12:00 pm, ET on 14 Sep 2018. Email quotes to olayemi.o.olatunji.mil@mail.mil. Questions are due by 3:00 pm ET, 12 Sep 2018, and must be submitted via email. Questions will not be answered via telephone. Questions that occur after the cut off, but are considered to be significant to the requirement, will be accepted and discussed only with those vendors that have submitted a quote. 6. QUOTATION PREPARATION INSTRUCTIONS: The Government will evaluate the quotes based on price, technical approach and if there is any negative information within the past three years on FAPIIS. Technical and performance capability will be rated as acceptable or unacceptable. (1) Contact/Company Information • point-of-contact name, address, e-mail, and telephone number • CAGE code and DUNS number The contracting officer is not responsible for locating or obtaining any information not identified in the quote. (2) Pricing • Price quotes must be firm-fixed price. • Include the total price (i.e., sum of all Option Years) in the price quote. • Terms are Net 30. • Prices (Quotes) must be valid for 60 days. • Contractor will submit invoice(s) through Wide Area Work Flow. The Government may not consider quotes that fail to include all of the information requested in this solicitation. The following provisions and clauses are applicable to this solicitation: 7. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. See attached addenda for quote instructions. Offers should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. See addenda. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. See addenda. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The additional FAR clauses cited in the clause are applicable to the acquisition. See addenda. Refer to the attachment 2 ( Med Surv. Clauses, 11 pages) for additional applicable clauses to be included in the award (details can be located on http://farsite.hill.af.mil. 8. Service Contract Act(SCA) are applicable. Current prevailing wage determination(s) at time of issuance of solicitation is WD15-4858 (7/03/18). Applicable wage rates can be found at http://www.wdol.gov. In the event the current prevailing Wage Determination is changed by the Department of Labor prior to the closing date of this solicitation, it is the offeror's responsibility to insure the most recent determination is used in preparing the quote. Should the wage rate change after closing, the offeror will be given a reasonable opportunity to provide revised quote solely to amend quote due to Wage Determination changes.
