This is a solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This solicitation is a request for quotations (RFQ). This solicitation for commercial services is in accordance with Federal Acquisition Regulation (FAR) part 12, Acquisition of Commercial Items and FAR part 13, Simplified Acquisition Procedures. The incorporated clauses and provisions are those in effect through Federal Acquisition Circular 2005-100. The associated North American Industrial Classification System (NAICS) code for this procurement is 325120, with a small business size standard of 1,000 employees. Period of performance for the base year shall begin on 10/01/2018 and end on 09/30/2019. Option year 1: 10/01/2019-09/30/2020 Option year 2: 10/01/2020-09/30/2021 Option year 3: 10/01/2021-09/30/2022 Option year 4: 10/01/2022-09/30/2023 Award shall be made to the offeror whose quotation offers the best value to the government, considering the following evaluation criteria: Lowest Price Technically Acceptable (LPTA) Only Please see the Solicitation Notice of Tiered Evaluations and Gray Market Prevention in Section D. To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must Meet the salient physical, functional, or performance characteristic specified in this solicitation; Clearly identify the item by: Brand name, if any; and Make or model number NOTE: OFFERORS MUST PROVIDE DESCRIPTIVE LITERATURE, TO INCLUDE CURRENT WARRANTY PROVISIONS TO SUPPORT THE GOVERNMENT S EVALUATION OF THE OFFERED PRODUCT(S). (See Clauses FAR 52.211-6 Brand Name or Equal and VAAR 852.211-73 Brand Name or Equal) Prospective offerors must be registered with the System for Award Management (SAM) website and complete representations and certifications at https://www.sam.gov. *NOTE* SDVOSBs and VOSBs must be verified for ownership and control and listed in the Vendor Information Pages at https://www.vip.vetbiz.gov when quotes are submitted and at the time of award. (See Clause 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside.) Offers received from concerns that are not Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns shall not be considered for award and will not be evaluated. The Government intends to award a base plus four option years, firm-fixed price services contract for Bulk oxygen as a result of this solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." Quotes are due at 10:00 A.M. EST on September 27, 2018. All quote submissions shall be submitted through e-mail only. No quotes will be accepted through phone or fax. No late submissions of quotation(s) will be accepted. Quotes shall be submitted on company letterhead. Commercial format is encouraged.
