- Statement of Work Attachment 1 Solicitation Package Subject: REQUEST FOR QUOTE Contractors are hereby requested to submit a price quote, for 150,000 Tarps in response to the attached Statement of Work and Tarp Specification Sheet. The Tarps will be distributed to the below Distribution Centers on Page 2. The 20,000 Tarps shall be delivered within seven days after award in order to provide resources to the disaster survivors in the affected areas. The remaining tarps must be delivered within 90 days from date of award. In response to the Quote, the following must be submitted for quotes to be accepted.: (1) Trade Agreement Certificate in accordance with FAR 52.225-6, confirming that your company is in compliance with TAA and certified for Fire Retardant; (2) A Letter of Commitment from the manufacturer on the company letterhead with contact information; and (3) The physical address(es) (including the street address) of the facilities where the tarps will be or were manufactured; (4) Certification of Origin submitted with invoices; Provide your approach for ensuring the commodities are delivered timely with the understanding of the Distribution Center(s) hours of operations. Please submit an electronic copy of your best and final price quote to Isaac L. Chapple at Isaac.Chapple@fema.dhs.gov, Lashawn Smith at Lashawn.Smith@fema.dhs.gov, FEMA-NRCC-CAS@FEMA.DHS.GOV, and FEMA-NRCC-CAUL@FEMA.DHS.GOV by Friday, September 14, 2018 at 10:00 AM, EST.
